血清VEGF-C及VEGFR-3水平对食管鳞癌放疗敏感性的预测价值

摘　　要：目的探讨血清血管内皮生长因子-C(VEGF-C)及血管内皮生长因子受体-3(VEGFR-3)水平对食管鳞癌放疗敏感性的预测价值。方法选取2017年6月至2019年11月在本院行放疗治疗的96例食管鳞癌患者作为癌症组,另选同期本院85名体检健康者作为健康组,均行血清VEGF-C、VEGFR-3水平检测。根据放疗效果将癌症组进一步分为有效组与无效组,比较两组的血清VEGF-C、VEGFR-3水平,同时,采用ROC曲线分析血清VEGF-C、VEGFR-3水平对食管鳞癌放疗敏感性的预测价值。结果癌症组的VEGF-C、VEGFR-3水平明显高于健康组(P<0.05)。有效组的血清VEGF-C、VEGFR-3水平明显低于无效组(P<0.05)。血清VEGF-C、VEGFR-3水平单一及联合检测预测食管鳞癌放疗敏感性的灵敏度分别为77.42%、74.19%、96.77%,特异度分别为83.08%、84.62%、80.00%,曲线下面积(AUC)分别为0.782、0.717、0.955。结论血清VEGF-C、VEGFR-3水平在食管鳞癌患者中异常升高,二者均对食管鳞癌放疗敏感性具有一定的预测价值,但联合检测的预测价值更高。Objective To investigate the predictive value of serum vascular endothelial growth factor-C(VEGF-C)and vascular endothelial growth factor receptor-3(VEGF-3)levels on the radiosensitivity of esophageal squamous cell carcinoma.Methods A total of 96 patients with esophageal squamous cell carcinoma treated by radiotherapy in our hospital from June 2017 to November 2019 were selected as cancer group,85 healthy people were selected as health group in the same period,and serum VEGF-C and VEGFR-3 levels were detected.According to the radiotherapy effect,the cancer group was further divided into effective group and invalid group,and the serum VEGF-C and VEGFR-3 levels were compared between the two groups.Meanwhile,the predictive value of serum VEGF-C and VEGFR-3 levels in the radiosensitivity of esophageal squamous cell carcinoma was analyzed by ROC curve.Results The levels of VEGF-C and VEGFR-3 in the cancer group were significantly higher than those in the healthy group(P<0.05).The serum VEGF-C and VEGFR-3 levels in the effective group were significantly lower than those in the invalid group(P<0.05).The sensitivity of serum VEGF-C and VEGFR-3 levels single and combined detection predicting radiosensitivity of esophageal squamous cell carcinoma was 77.42%,74.19%and 96.77%respectively,the specificity was 83.08%,84.62%and 80.00%respectively,and the area under curve(AUC)was 0.782,0.717 and 0.955 respectively.Conclusion The serum VEGF-C and VEGFR-3 levels of patients with esophageal squamous cell carcinoma abnormally increased.Both of them have certain predictive value on the radiosensitivity of esophageal squamous cell carcinoma,but the combined detection has higher predictive value.
