胃癌D2根治术采用腹腔镜与传统开腹术治疗进展期胃癌的效果评价

摘　　要：目的探究进展期胃癌患者实施腹腔镜胃癌D2根治术的效果。方法选取2017年10月至2018年10月于本院行胃癌D2根治术治疗的进展期胃癌患者82例,采用抽签法分为实验组与参照组,每组41例。参照组采用传统开腹式胃癌D2根治术,实验组行腹腔镜胃癌D2根治术,比较两组患者手术相关指标及并发症情况。结果两组患者淋巴清扫数比较差异无统计学意义;与参照组比较,实验组手术时间、排气时间、住院时间及切口长度较短,且出血量较低,差异均有统计学意义(P<0.05)。实验组肺炎、切口感染、吻合口漏、肠梗阻等并发症发生率为7.32%,低于参照组的29.27%,组间比较差异有统计学意义(P<0.05)。结论对进展期胃癌患者分别采取腹腔镜下胃癌D2根治术、传统开腹术,两种术式效果相当,但腹腔镜下胃癌D2根治术能够减少患者术后并发症,保证患者临床安全,值得临床推广。Objective To study the effect of laparoscopic gastric cancer D2 radical surgery in patients with advanced gastric cancer.Methods 82 patients with advanced gastric cancer who were treated with gastric cancer D2 radical surgery in our hospital were selected from October 2017 to October 2018.They were divided into experimental group and reference group by the lottery method,with 41 cases in each group.The reference group used traditional open radical D2 radical gastrectomy for gastric cancer,the experimental group underwent laparoscopic gastric cancer D2 radical surgery,and the surgical indicators and complications were compared between the two groups.Results There was no significant difference in the number of lymphatic dissections between the two groups.Compared with the reference group,the length of the surgery,the duration of the exhaust,and the length of the hospital stay and the length of the incision were shorter,and the amount of bleeding were lower,the difference was significant(P<0.05).In terms of treatment safety,the incidence of complications such as pneumonia,incision infection,anastomotic leakage,and intestinal obstruction in the experimental group(7.32%)was lower than that in the reference group(29.27%),there was significant difference between the two groups(P<0.05).Conclusion Laparoscopic gastric cancer D2 radical surgery and traditional laparotomy are performed for patients with advanced gastric cancer.The two methods are equivalent,but laparoscopic gastric cancer D2 radical surgery can further reduce postoperative complications and ensure clinical safety of patients.It should be clinically promoted.
