摘 要:目的研究单核细胞/高密度脂蛋白比值(MHR)对急性脑梗死患者颅内外动脉粥样硬化性狭窄的预测价值。方法回顾性收集青海省人民医院住院的急性脑梗死患者181例,根据颅内外动脉粥样硬化性狭窄程度分为对照组(狭窄率<50%)77例和狭窄组(狭窄率≥50%)104例,对比分析2组临床基线资料及各项指标。采用logistic回归模型进行危险因素分析。采用ROC曲线分析MHR对颅内外动脉粥样硬化狭窄的预测作用。结果狭窄组单核细胞、MHR、TG、白细胞计数、血小板计数、C反应蛋白水平明显高于对照组,HDL-C水平明显低于对照组(P<0.05,P<0.01)。MHR(95%CI:2.981~205.092,P=0.006)、白细胞计数(95%CI:1.199~1.772,P=0.000)、血小板计数(95%CI:1.001~1.012,P=0.014)为急性脑梗死患者颅内外动脉粥样硬化性狭窄的危险因素。ROC曲线分析显示,MHR对颅内外动脉粥样硬化狭窄预测的曲线下面积为0.783(95%CI:0.716~0.851,P=0.000)。结论MHR对急性脑梗死患者颅内外动脉粥样硬化性狭窄具有一定预测价值。Objective To study the value of monocyte/HDL ratio(MHR)in predicting intracranial and extracranial atherosclerosis in acute cerebral infarction(ACI)patients.Methods One hundred and eighty-one ACI patients admitted to our hospital were divided into control group(n=77)with an intracranial and extracranial atherosclerotic stenosis rate<50%and stenosis group(n=104)with an intracranial and extracranial atherosclerotic stenosis rate≥50%.Their baseline clinical data and laboratory testing parameters were comparatively analyzed.The risk factors for intracranial and extracranial atherosclerotic stenosis were analyzed by logistic regression analysis.The value of MHR in predicting intracranial and extracranial atherosclerosis was analyzed by ROC curve analysis.Results The number of monocytes,WBC and platelets was significantly smaller,the MHR and serum TG and CRP levels were significantly higher while the serum HDL-C level was significantly lower in stenosis group than in control group(P<0.05,P<0.01).The MHR and the number of WBC and platelets were the risk factors for intracranial and extracranial atherosclerotic stenosis in ACI patients(OR=24.71,95%CI:2.981-205.092,P=0.006;OR=1.46,95%CI:1.199-1.772,P=0.000;OR=1.01,95%CI:1.001-1.012,P=0.014).ROC curve analysis showed that the AUC for MHR in predicting intracranial and extracranial atherosclerotic stenosis was 0.783(95%CI:0.71-60.851,P=0.000).Conclusion MHR can predict intracranial and extracranial atherosclerosis in ACI patients.
