细胞外信号调节激酶1/2在肾癌中的表达及临床意义

摘　　要：目的:探讨细胞外信号调节激酶1/2(ERK1/2)及其磷酸化状态(p-ERK1/2)在肾癌中的表达及临床意义。方法:选取78例肾癌患者的肾癌组织及距离病灶超过3 cm的癌旁组织,免疫组织化学法检测ERK1/2及p-ERK1/2表达情况,比较性别、年龄、肿瘤直径、临床分期、淋巴结转移、远处转移、病理Fuhrman分级与癌组织ERK1/2及p-ERK1/2的关系,并通过Kaplan-Meier生存分析癌组织中ERK1/2、p-ERK1/2与肾癌患者生存情况的相关性。结果:肾癌组织中ERK1/2及p-ERK1/2阳性表达率均高于癌旁组织(P<0.05)。不同性别、年龄、肿瘤直径患者癌组织ERK1/2及p-ERK1/2阳性表达率之间比较,差异无统计学意义(P>0.05);临床分期为Ⅲ-Ⅳ期、有淋巴结转移、远处转移、病理Fuhrman分级为G2+G3者的癌组织中ERK1/2及p-ERK1/2阳性表达率分别高于临床分期Ⅰ-Ⅱ期、无淋巴结转移、远处转移、病理Fuhrman分级为G1者(P<0.05)。Kaplan-Meier生存分析结果显示,相较于ERK1/2、p-ERK1/2阴性表达者,肾癌组织中ERK1/2、p-ERK1/2阳性表达的患者生存情况较差(P<0.001)。结论:ERK1/2及p-ERK1/2在肾癌组织高表达,且与患者临床分期、淋巴结转移、远处转移、病理Fuhrman分级及生存期相关。Objective:To investigate the expression and clinical significance of extracellular signal-regulated kinase 1/2(ERK1/2)and its phosphorylation state(p-ERK1/2)in renal carcinoma.Methods:The resected renal cell carcinoma and the adjacent tissue more than 3 cm away from the lesion were taken from 78 patients with renal cell carcinoma.The expressions of ERK1/2 and p-ERK1/2 in different tissues were detected by immunohistochemistry(three-step method),and their relationship with different gender,age,tumor diameter,clinical stage,lymph node metastasis,distant metastasis and pathological Fuhrman grade were compared.The correlation between the expression of ERK1/2,p-ERK1/2 and the survival of patients with renal cell carcinoma was also analyzed by Kaplan Meier survival analysis.Results:The positive expression rates of ERK1/2 and p-ERK1/2 in cancer tissue were significantly higher than those in adjacent tissues(P<0.05).There was no significant difference in the positive rates of ERK1/2 and p-ERK1/2 between patients with different gender,age and tumor diameter(P>0.05).The positive expression rates of ERK1/2 and p-ERK1/2 in patients with clinical stage III-IV,lymph node metastasis,distant metastasis and pathological Fuhrman grade G2+G3 were obviously higher than those with clinical stage I-II,no lymph node metastasis,no distant metastasis,pathological Fuhrman grade G1,respectively(P<0.05).Kaplan Meier survival analysis showed that compared with the negative expression of ERK1/2 and p-ERK1/2,the survival of patients with positive expression of ERK1/2 and p-ERK1/2 was worse(P<0.001).Conclusion:ERK1/2 and p-ERK1/2 are highly expressed in renal carcinoma tissue,and their expression levels are correlated with clinical stage,lymph node metastasis,distant metastasis,pathological Fuhrman grade and survival time.
