内镜下注射高渗钠-肾上腺素盐水联合奥曲肽治疗非静脉曲张上消化道出血的效果观察

摘　　要：目的:研究内镜下注射高渗钠-肾上腺素盐水联合奥曲肽治疗非静脉曲张上消化道出血(non-variceal upper gastrointestinal hemorrhage,NVUGIH)的临床效果。方法:选取2019年4月-2020年4月本院收治的60例NVUGIH患者,按随机数字表法将其分为观察组和对照组,各30例。对照组给予内镜下注射高渗钠-肾上腺素盐水治疗,观察组在对照组基础上联合奥曲肽治疗。比较两组患者止血成功率、再出血率、总有效率,治疗前后的出血相关指标变化及不良反应发生情况。结果:观察组的止血成功率和再出血率均优于对照组,差异均有统计学意义(P<0.05)。治疗3 d,观察组的RBC、Hb水平均高于对照组,PT、RTC、BUN水平均低于对照组,差异均有统计学意义(P<0.05)。观察组的总有效率高于对照组,差异有统计学意义(P<0.05)。两组用药期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:内镜下注射高渗钠-肾上腺素盐水联合奥曲肽治疗NVUGIH患者的效果确切,可有效提高止血成功率,降低再出血率,改善患者的出血相关指标,安全性较好,值得临床推广应用。Objective:To study the clinical effect of endoscopic injection of Hypertonic Sodium-Epinephrine Saline combined with Octreotide on non-variceal upper gastrointestinal hemorrhage(NVUGIH).Method:A total of 60 NVUGIH patients admitted to our hospital from April 2019 to April 2020 were selected,they were divided into observation group and control group according to random number table method,with 30 cases in each group.The control group was given endoscopic injection of Hypertonic Sodium-Adrenaline Saline,the observation group was treated with Octreotide on the basis of the control group.Compared the hemostatic success rate,rebleeding rate,total effective rate,changes in bleeding-related indicators before and after treatment and the occurrence of adverse reactions in the two groups.Result:The hemostatic success rate and rebleeding rate of the observation group were better than those of the control group,the differences were statistically significant(P<0.05).After treatment for 3 d,the RBC,Hb levels in the observation group were all higher than those in the control group,while PT,RTC and BUN levels were all lower than those in the control group,with statistically significant differences(P<0.05).The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Endoscopic injection of Hypertonic Sodium-Adrenaline Saline combined with Octreotide in the treatment of patients with NVUGIH has definite curative effect,which can effectively improve the success rate of hemostasis and reduce rebleeding rate,improve the bleeding-related indicators of patients,it has good safety,it is worthy of clinical application.
