《针灸大成》中尺泽穴临床应用探析

摘　　要：《针灸大成》记载,尺泽穴为手太阴肺经合穴,既有肺经腧穴的共性,又有五输穴合穴的特性,具有养阴润肺、清泄肺热、条畅气机、化痰止咳平喘、疏经活络、通络止痛、止血等功效,常用于治疗肺系疾病、肢体经脉痛证、脾胃疾病、神志病及小儿慢惊风等。取穴方法,既可单独使用,也可辨证配伍其他穴位。治疗小儿慢惊风时,单独使用此穴即可见效。治疗肺系疾病时,多配本经的列缺、经渠、鱼际等穴。治疗肘臂拘挛时,常以局部取穴为主,配伍曲池、合谷等穴。治疗脾胃疾病时,根据循经取穴,配伍足三里、三阴交、太白等穴。操作手法上,强调补泻手法的运用,尺泽穴为肺经合穴,经脉气血汇聚之处,五行属水,临床上多采用强刺激泻法以清泄肺热,和胃理气。According to Great Compendium of Acupuncture and Moxibustion,Chize(LU 5)is the sea point of lung meridian of hand greater yin,not only has the common characteristics of lung meridian points,but also has the characteristics of five transport points.It has the functions of nourishing yin and moistening lung,clearing lung heat,smoothing qi movement,resolving phlegm,relieving cough and calming asthma,activating collaterals,dredging meridians,relieving pain and stopping bleeding.It is commonly used in the treatment of lung diseases,limb meridian pain syndrome,spleen and stomach diseases,mental diseases and children's chronic convulsion.The point selection method can be used alone or combined with other points.In the treatment of children with chronic convulsion,the use of this point alone can be effective.In the treatment of lung diseases,Lieque(LU 7),Jingqu(LU 8)and Yuji(LU 10)of the lung meridian are often used.In the treatment of elbow and arm contracture,local points are often selected,combined with Quchi(LI 11),Hegu(LI 4)and others.In the treatment of spleen and stomach diseases,points should be selected according to the meridians and combined with Zusanli(ST 36),Sanyinjiao(SP 6),Taibai(SP 3)and other points.In terms of operation manipulation,the application of reinforcing reducing technique is emphasized.Chize(LU 5)is the sea point of lung meridian,where qi and blood gather.It belongs to water according to the five elements.In clinical practice,strong stimulation reducing method is often used to clear lung heat and regulate stomach and regulate qi.
