摘  要:目的探索乌司他丁对脑缺血再灌注大鼠海马N-甲基-D-天门冬氨酸受体(NMDAR)表达的影响。方法将72只雄性SD大鼠随机分为假手术组(S组)、模型组(I/R组)和乌司他丁干预组(U组)。采用线栓法阻断大脑右侧中动脉建立脑缺血损伤模型。U组于再灌注即刻腹腔注射乌司他丁20000 U/kg,再灌注3 h后进行神经功能评分(NDS),采用TUNEL法检测脑组织凋亡细胞数,RT-PCR检测NMDAR1 mRNA的表达水平,采用免疫组化染色计算平均光密度值(MOD)观察NMDAR1的表达。结果与S组(0)比较,I/R组(2.58±0.35)和U组(1.34±0.41)的NDS升高(P<0.05);与S组(0)比较,I/R组(52.10±3.25)和U组(26.40±2.70)的凋亡细胞数增加(P<0.05);与S组(0.8902±0.0136)比较,I/R组(1.0398±0.0211)和U组(0.9073±0.0142)NMDAR1 mRNA的表达上调(P<0.05);与S组(0.0066±0.0007)比较,I/R组(0.0594±0.0111)和U组(0.0068±0.0004)NMDAR1表达的MOD值升高(P<0.05)。与I/R组(2.58±0.35)比较,U组(1.34±0.41)的NDS降低(P<0.05);与I/R组(52.10±3.25)比较,U组(26.40±2.70)凋亡细胞数减少(P<0.05);与I/R组(1.0398±0.0211)比较,U组(0.9073±0.0142)NMDAR1 mRNA的表达下降(P<0.05);与I/R组(0.0594±0.0111)比较,U组(0.0068±0.0004)NMDAR1表达的MOD值降低(P<0.05)。结论乌司他丁减轻脑缺血再灌注损伤的机制可能与抑制NMDAR1表达有关。Objective To explore the effect of ulinastatin on the expression of N-methyl-D-aspartate receptor(NMDAR)in hip⁃pocampus of rats with cerebral ischemia reperfusion injury.Methods Totally 72 male SD rats were randomly divided into sham opera⁃tion group(S group),model group(I/R group)and ulinastatin intervention group(U group).The model of cerebral ischemia injury was established by blocking the right middle cerebral artery with wire plug.The rats of U group was intraperitoneally injected with ulinastatin 20000U/kg immediately after perfusion.The eurological function scores were performed 3h after reperfusion,the number of apoptotic cells in brain tissue was measured by TUNEL assay,the expression level of NMDAR1mRNA was detected by RT-PCR.Immunohistochemical staining was used to calculate the mean optical density value(MOD)to observe the expression of NMDAR1.Results Compared with S group(0),NDS in I/R group(2.58±0.35)and U group(1.34±0.41)were increased(P<0.05).Compared with S group(0),the number of apoptotic cells in I/R group(52.10±3.25)and U group(26.40±2.70)were increased(P<0.05).Compared with S group(0.8902±0.0136),the expression of NMDAR1 mRNA in I/R group(1.04±0.02)and U group(0.91±0.01)was up-regulated(P<0.05).Com⁃pared with S group(0.0066±0.0007),the MOD value of NMDAR1 expression in I/R group(0.0594±0.0111)and U group(0.0068±0.0004)was increased(P<0.05).Compared with the I/R group(2.58±0.35),the NDS of the U group(1.34±0.41)decreased(P<0.05).Compared with the I/R group(52.10±3.25),the number of apoptotic cells of the U group(26.4±2.70)decreased(P<0.05).Com⁃pared with the I/R group(1.0398±0.0211),the expression of NMDAR1 mRNA in the U group(0.9073±0.0142)decreased(P<0.05),compared with the I/R group(0.0594±0.0111).The MOD value of NMDAR1 expression in U group(0.0068±0.0004)was de⁃creased(P<0.05).Conclusion The mechanism of ulinastatin attenuating cerebral ischemia-reperfusion injury may be related to the in⁃hibition of NMDAR1 expression.
