多学科协作下的ERAS理念在老年髋骨骨折患者围手术期护理中的应用效果

摘　　要：目的探讨多学科协作下的加速康复外科(ERAS)理念在老年髋骨骨折患者围手术期护理中的应用效果。方法选取122例老年髋骨骨折患者为研究对象,采用随机数字表法将其分为对照组和观察组,各61例。对照组给予常规围术期护理干预,观察组在对照组基础上给予多学科协作下的ERAS理念干预。比较两组的护理干预效果。结果观察组的住院天数、首次下床活动时间均短于对照组(P<0.05)。术后1、2周,观察组的VAS、PSQI评分低于对照组,Harris、Barthel评分高于对照组(P<0.05)。术后1周,观察组的HAMA评分低于对照组(P<0.05)。观察组术后并发症总发生率低于对照组(P<0.05)。结论多学科协作下的ERAS理念在老年髋骨骨折患者围手术期护理中的应用效果显著,可提高患者睡眠质量及自我护理能力,改善髋关节功能,减少并发症。Objective To explore the application effect of enhanced recovery after surgery(ERAS)concept under multidisciplinary cooperation on perioperative nursing of elderly patients with hip fracture.Methods A total of 122 elderly patients with hip fracture were selected as the study objects,and the patients were divided into control group and observation group by random number table method,with 61 cases in each group.The control group was given routine perioperative nursing intervention,and the observation group was given ERAS concept intervention under multidisciplinary cooperation on the basis of the control group.The effect of nursing intervention was compared between the two groups.Results The hospitalization days and the first time out of bed activity time of the observation group were shorter than those of the control group(P<0.05).At 1 and 2 weeks after operaiton,VAS and PSQI scores of the observation group were lower than those of the control group,while Harris and Barthel scores of the observation group were higher than those of the control group(P<0.05).At 1 week after operation,the HAMA score of the observation group was lower than that of the control group(P<0.05).The total incidence of postoperative complications of the observation group was lower than that of the control group(P<0.05).Conclusion The ERAS concept under multidisciplinary cooperation in perioperative nursing of elderly patients with hip fracture has a significant application effect,which can improve the quality of sleep and self-care,ameliorate hip joint function,and reduce complications.
