摘 要:目的探讨光学增强内镜(optical enhancment,OE)对喉癌前病变及喉癌诊断的初步应用价值。方法选取2018年5月~2019年12月检查发现喉肿物患者共121例(162侧),应用光学增强内镜的3种观察模式(普通白光模式、OE模式1、OE模式2)进行喉部病变观察,观察喉部病变两种OE模式下表现及其与病理结果之间的关系。结果两种OE模式诊断声带息肉、喉癌前病变及喉癌准确性均高于普通白光模式,差异均有统计学意义(P均<0.05);OE模式1、OE模式2对喉癌前病变的诊断符合率分别为95.35%、97.67%及检出率分别为85.07%、94.03%,均高于普通白光模式58.14%、44.78%,差异均有统计学意义(P均<0.01)。结论光学增强内镜两种OE模式可提高喉癌前病变及喉癌正确诊断率,OE模式2兼具观察黏膜微血管及凸显病变黏膜自然原色的特性,利于病变早期检出并诊断。OBJECTIVE To explore the preliminary application of optical enhanced endoscopy in the diagnosis of premalignant and malignant laryngeal lesions.METHODSFrom May 2018 to December 2019,a total of 121 patients with laryngeal tumors were diagnosed(162 lesions).Three observation modes(white light mode,OE mode 1,OE mode 2)of optically enhanced endoscopy were used to observe laryngeal lesions,and study the manifestations of laryngeal lesions under two OE modes and their relationship with pathological results.RESULTS The accuracy of the two OE modes in diagnosing vocal cord polyps,premalignant lesions and malignant lesions were higher than that of white light mode,the differences were statistically significant(P<0.05).The consistency rate(95.35%and 97.67%)and detection rates(85.07%and 94.03%)of the two OE modes were higher than those of the white light mode(58.14%and 44.78%),the differences were statistically significant(P<0.01).CONCLUSION Two OE modes can improve the correct diagnosis rate of premalignant and malignant lesions,and have advantages in the detection and diagnosis of premalignant lesions.OE mode 2 highlights the microvascular morphology of the diseased mucosa while presenting it with a natural color,and is useful for early detection and diagnosis.
