摘  要:目的探讨口腔鳞状细胞癌患者唾液中癌胚抗原(CEA)、过氧化氢酶(CAT)及细胞角质蛋白19片段抗原21-1(CYFRA21-1)的表达水平及临床意义。方法选取84例口腔鳞状细胞癌患者、50例口腔良性肿瘤患者及50例体检健康者,分别作为口腔鳞状细胞癌组、良性肿瘤组和健康对照组,检测3组研究对象唾液中CEA、CAT及CYFRA21-1水平,比较不同临床特征口腔鳞状细胞癌患者唾液中CEA、CAT及CYFRA21-1的水平。采用受试者工作特征(ROC)曲线分析唾液中CEA、CAT及CYFRA21-1水平对口腔鳞状细胞癌的诊断价值。结果口腔鳞状细胞癌组、良性肿瘤组患者唾液中CEA、CAT及CYFRA21-1水平均高于健康对照组,差异均有统计学意义(P﹤0.05);口腔鳞状细胞癌组患者唾液中CEA、CAT及CYFRA21-1水平均高于良性肿瘤组,差异均有统计学意义(P﹤0.05)。临床分期为Ⅲ~Ⅳ期、有淋巴结转移的口腔鳞状细胞癌患者唾液中CEA、CYFRA21-1水平均明显高于临床分期为Ⅰ~Ⅱ期、无淋巴结转移的患者,差异均有统计学意义(P﹤0.01)。唾液中CEA、CAT及CYFRA21-1水平诊断口腔鳞状细胞癌的ROC曲线下面积分别为0.790、0.750和0.896,P﹤0.05,截断值分别为38.50 ng/ml、46.50 U/ml和50.20μg/L,灵敏度分别为72.20%、64.50%和80.00%,特异度分别为65.50%、61.00%和78.50%。结论口腔鳞状细胞癌患者唾液中CEA、CAT及CYFRA21-1水平均升高,CEA和CYFRA21-1水平与患者的临床分期及淋巴结转移情况有关。Objective To investigate the expression of carcinoembryonic antigen(CEA),catalase(CAT),and cytokeratin 19 fragment antigen 21-1(CYFRA21-1)in the saliva of patients with oral squamous cell carcinoma and their clinical significance.Method A total of 84 patients with oral squamous cell carcinoma,50 patients with benign oral tumor,and 50 healthy subjects were selected and divided into the oral squamous cell carcinoma group,benign tumor group,and healthy control group,respectively.The saliva CEA,CAT,and CYFRA21-1 levels were detected in all groups.Levels of CEA,CAT,and CYFRA21-1 in the saliva of patients with oral squamous cell carcinoma with different clinical characteristics were compared.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of saliva CEA,CAT,and CYFRA21-1 levels in patients with oral squamous cell carcinoma.Result The saliva levels of CEA,CAT,and CYFRA21-1 in the oral squamous cell carcinoma group and the benign tumor group were higher than those in the healthy control group,and the differences were statistically significant(P<0.05).The saliva levels of CEA,CAT,and CYFRA21-1 in the oral squamous cell carcinoma group were higher than those in the benign tumor group,and the differences were statistically significant(P<0.05).The levels of CEA and CYFRA21-1 in the saliva of oral squamous cell carcinoma patients with clinical stage Ⅲ-Ⅳ and lymph node metastasis were significantly higher than those of patients with clinical stage Ⅰ-Ⅱ and without lymph node metastasis,and the differences were statistically significant(P<0.01).The area under the ROC curve of CEA,CAT,and CYFRA21-1 levels in saliva for the diagnosis of oral squamous cell carcinoma was 0.790,0.750,and 0.896(P<0.05),the cut-off values were 38.50 ng/ml,46.50 U/ml,and 50.20μg/L,the sensitivity were 72.20%,64.50%,and 80.00%,and the specificity were 65.50%,61.00%,and 78.50%,respectively.Conclusion CEA,CAT and CYFRA21-1 in the saliva of patients with oral squamous cell carcinoma are up-regulation,and the
