异体血管移植技术在门静脉/肠系膜上静脉受侵犯的胰腺癌根治术中的应用价值

摘　　要：目的探讨异体血管移植技术在门静脉(PV)/肠系膜上静脉(SMV)受侵犯的胰腺癌根治术中的应用价值。方法根据手术方式的不同将84例PV/SMV受侵犯的胰腺癌患者分为A组(n=50)和B组(n=34),两组患者均进行胰腺癌根治术,其中A组患者采取异体血管移植技术,B组患者采取姑息性旁路手术。比较两组患者的手术情况、术后并发症发生情况及生存情况。结果A组患者的手术时间为(427.0±89.0)min,明显长于B组患者的(117.0±34.0)min,差异有统计学意义(P﹤0.01);A组患者的术中出血量为(975.0±409.0)ml,明显多于B组患者的(139.0±39.0)ml,差异有统计学意义(P﹤0.01)。A组和B组患者术后各并发症发生率及并发症总发生率比较,差异均无统计学意义(P﹥0.05)。A组患者的中位生存时间为11.83个月,长于B组患者的7.50个月,差异有统计学意义(P﹤0.05)。结论异体血管移植技术在PV/SMV受侵犯的胰腺癌根治术中可获得理想的手术效果,能够明显延长患者的生存时间。Objective To explore the application value of heterogeneous vascular transplantation in the treatment of pancreatic cancer with portal vein(PV)/superior mesenteric vein(SMV)involvement.Method 84 pancreatic cancer patients with PV/SMV involvement were divided into group A(n=50,treated with heterogeneous vascular transplantation)and group B(n=34,treated with palliative bypass surgery).The surgery condition,post-surgical complications and survival were compared between the two groups.Result The operation time in group A was(427.0±89.0)min,which was significantly longer than that of(117.0±34.0)min in group B(P<0.01).The intra-operative blood loss in group A was(975.0±409.0)ml,which was significantly higher than that of(139.0±39.0)ml in group B(P<0.01).There were no significant differences for the incidence of overall postoperative complications and individual complications between the two groups(P>0.05).The median survival time of group A(11.83 months)was longer than that of group B(7.50 months)(P<0.05).Conclusion Heterogeneous vascular transplantation in treatment of pancreatic cancer with PV/SMV involvement can achieve favorable outcome,and significantly prolong the survival time of patients.
