Bcl-2蛋白表达在卵巢巧克力囊肿患者术后妊娠预后的价值分析

摘　　要：目的探讨细胞凋亡抑制因子(Bcl-2)蛋白表达水平在卵巢巧克力囊肿患者术后妊娠预后的价值。方法选取2014年1月—2016年8月在我院进行腹腔镜手术治疗的82例卵巢巧克力囊肿患者以及82例卵巢存档组织,分别作为研究组和对照组。比较两组研究对象的卵巢组织Bcl-2蛋白表达水平,并对研究组患者术后2年妊娠情况进行随访;分析Bcl-2蛋白表达水平与患者术后妊娠率之间的关系以及影响患者术后妊娠预后独立影响因素。结果研究组卵巢囊肿组织Bcl-2蛋白水平相比对照组显著增高(P<0.001),且研究组患者Bcl-2 mRNA表达量显著高于对照组(P<0.001)。78例患者中术后两年的总体妊娠率为60.26%(47/78);其中Bcl-2蛋白表达水平为(–)、(+)、(++)、(+++)患者的两年妊娠率分别为100%、71.43%、63.64%和35.00%,差异具有统计学意义(P<0.05);COX回归分析结果显示不孕时长、最低功能(LF)评分、Bcl-2蛋白水平是影响患者术后妊娠预后的独立影响因素。结论卵巢巧克力囊肿患者卵巢组织Bcl-2蛋白表达水平显著高于健康人群,且其表达水平对患者行腹腔镜手术治疗后短妊娠预后具有较高的预测价值。Objective To explore the value of Bcl-2 protein expression in pregnancy outcome of patients with ovarian chocolate cysts. Methods From January 2014 to August 2016, 82 patients with ovarian chocolate cysts who underwent laparoscopic surgery in our hospital and 82 archival ovarian tissue served as the study group and control group. The expression levels of Bcl-2 protein in the ovarian tissue of the two groups of study subjects was compared, the follow-up of pregnancy in the study group was followed up for 2 years;the relationship between the expression of Bcl-2 protein and the postoperative pregnancy rate was analyzed, and the independent factors affecting the postoperative pregnancy prognosis were analyzed. Results The level of Bcl-2 protein in the ovarian cysts of the study group was significantly enhanced compared with the control group(P<0.001), and the expression of Bcl-2 mRNA in the patients of the study group was significantly higher than that of the control group(P<0.001). Among the 78 patients, the overall pregnancy rate was 60.26%(47/78) at two years after operation;Bcl-2 protein expression levels were(-),(+),(++),(+++). The annual pregnancy rates were 100%, 71.43%, 63.64%, and 35.00%, the differences were statistically significant(P<0.05);COX regression analysis showed that infertility duration, LF score, and Bcl-2 protein levels were independent factors that affected patients’ postoperative pregnancy prognosis. Conclusion The expression level of Bcl-2 protein in ovarian tissue of patients with ovarian chocolate cysts is significantly higher than that in healthy people, and its expression level has a higher predictive value for patients with short pregnancy after laparoscopic surgery.
