腔镜甲状腺手术后发生咽喉痛的影响因素研究

摘　　要：目的分析甲状腺手术患者术后发生咽喉痛的相关影响因素。方法选取2019年1月至2020年1月于武警湖北省总队医院行甲状腺手术的患者95例,依据术后是否合并咽喉痛分为两组,无疼痛者52例为对照组,有疼痛者43例为观察组,比较两组患者的临床资料及麻醉相关资料,采用多因素Logistic回归分析甲状腺术后并发咽喉痛的危险因素。结果两组患者在年龄、手术次数、手术方式、是否行颈部淋巴结清扫、麻醉持续时间、气管插管尝试次数及套囊压力等方面差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示年龄>60岁、二次手术、双侧颈部淋巴结清扫、多次尝试气管插管、套囊压力>26 mmHg为甲状腺手术后并发咽喉痛的危险因素。结论甲状腺手术可使患者并发咽喉痛,术中麻醉及手术手法应轻柔,术后加强监测并及时对症处理。Objective To investigate the factors related to laryngopharyngeal pain in patients undergoing thyroid surgery.Methods 95 cases of thyroid surgery patients from January 2019 to January 2020 in our hospital were retrospectively analyzed,on the basis of whether patients with postoperative merger throat pain,divided into control group(no pain,52 cases)and observation group(with pain,43 cases),and compared the clinical data of two groups of patients with thyroid surgery and anesthesia related factors.Thyroid surgery and a risk factor for a sore throat was used multiariable Logistic regression analysis.Results There were differences between the two groups in age,number of operations,surgical method,cervical lymph node dissection,duration of anesthesia,number of endotracheal intubation attempts,and cuff pressure(P<0.05).Multivariate Logistic regression analysis showed that the risk factors for laryngeal pain in patients undergoing thyroid surgery were>60 years of age,reoperation,bilateral cervical lymph node dissection,multiple attempts to intubate,and cuff pressure>26 mmHg.Conclusions Thyroid surgery can cause patients with laryngopharyngeal pain,anesthesia and operation should be gentle,postoperative monitoring should be strengthened,timely symptomatic treatment.
