摘  要:目的探讨右美托咪定对大鼠脑缺血再灌注损伤认知功能的影响及可能机制。方法 54只雄性SD大鼠随机分为假手术组、模型组和右美托咪定处理组,线栓法制备大鼠大脑缺血再灌注模型,对各组大鼠的神经功能障碍进行评分;检测各组大鼠脑梗死体积;免疫组织化学方法和Western blot方法检测各组大鼠海马组织高迁移率族蛋白B1(HMGB1)与Toll样受体4(TLR4)的表达;ELISA方法检测各组大鼠海马组织IL-1β、IL-6与TNF-α的水平变化。结果右美托咪定预处理能够明显改善脑缺血再灌注大鼠神经功能障碍、缩小脑梗死体积(P<0.05);下调海马组织HMGB1与TLR4的表达(P<0.05);下调海马组织炎性因子IL-1β、IL-6与TNF-α的表达水平(P<0.05)。结论右美托咪定能改善脑缺血再灌注大鼠的认知功能,其机制可能与抑制HMGB1/TLR-4炎性信号通路有关。Objective To investigate the effect of dexmedetomidine on cognitive function in rats with cerebral ischemia-reperfusion injury and its possible mechanism. Methods 54 male SD rats were randomly divided into sham group, model groupand dexmedetomidine treatment group. The neurological dysfunction of rats in each group was scored, and the cerebral infarction volume was detected. Immunohistochemistry and Western blot were used to detect the expression of high mobility group protein B1(HMGB1), toll like receptor 4(TLR4) and nuclear factor kappa B(NF-κB) in hippocampus of rats in each group.The levels of IL-1 β, IL-6 and TNF-α in hippocampus were detected by ELISA. Results Dexmedetomidine pretreatment can significantly improve the neurological dysfunction and reduce the volume of cerebral infarction in rats with cerebral ischemia-reperfusion(P<0.05);down regulate the expression of HMGB1, TLR4 and NF-κB in hippocampus(P<0.05);down regulate the levels of inflammatory factors IL-1 β, IL-6 and TNF-α in hippocampus(P<0.05). Conclusion Dexmedetomidine can improve the cognitive function of rats with cerebral ischemia-reperfusion, the mechanism might be related to the inhibition of HMGB1/TLR-4/NF-κB inflammatory signal pathway.
