摘　　要：目的:评价针状电极整块切除法治疗直径<3 cm的单发膀胱癌的疗效。方法:回顾性分析105例诊断为<3 cm的单发非肌层浸润性膀胱癌患者的临床资料,其中针状电极组55例,传统电切组50例。记录手术时间、膀胱冲洗时间,病检合格率、手术并发症及术后1年总复发率,并进行统计学分析。结果:两组手术时间比较差异无统计学意义(P>0.05)。针状电极组的标本合格率高于传统电切组,术后并发症、膀胱冲洗时间、二次电切率、术后1年总复发率均低于传统电切组。结论:针状电极行对直径<3 cm的膀胱肿瘤行整块切除是安全有效的。肿瘤整块切除提高了肿瘤标本的质量,降低了二次电切率。Objective:To evaluate the clinical efficacy of transurethral en-bloc resection of bladder tumour(ERBT)smaller than 3 cm in diameter by a needle electrode.Methods:A total of 105 patients who were diagnosed as non-muscle-invasive bladder cancer smaller than 3 cm in diameter were included in the retrospective study including 50 patients receiving conventional transurethral resection of bladder tumour(TURBT)and 55 patients receiving transurethral resection of bladder tumour enucleation by a needle electrode.The time of operation,postoperative irrigation,the passing rate of pathological examination,complications,and one-year recurrence rate of tumour were compared.Results:There was no significant difference in the operation time between the two groups(P>0.05).The complications,the time of postoperative irrigation,rate of the second TURBT and one-year recurrence rate of bladder tumour were lower in the group of ERBT than those in the group of TURBT.Conclusion:ERBT by a needle electrode is safe and effective for the treatment of bladder tumour smaller than 3 cm in diameter.Furthermore,ERBT improves the quality of tumour specimens and decreases the second TURBT.Further randomized controlled trials are needed to certify our results.
