摘　　要：目的探讨食管癌手工吻合术后早期肠内营养(early enteral nutrition,EEN)较延迟肠内营养(delayed enteral nutrition,DEN)对患者术后营养状况及肠道功能恢复的意义。方法回顾性分析本院106例食管癌手术患者的临床资料,分为EEN组(n=58)及DEN组(n=48),分别比较两组患者围术期一般状况、术后肠道功能恢复情况、EN并发症发生率、营养状况恢复及感染指标恢复情况。结果两组患者围术期一般状况比较差异无统计学意义,与DEN组相比,EEN组术后早期饥饿感较少、力量恢复评分较高、首次排气时间较短、精神状况评分较高,住院费用较少,差异有统计学意义(P<0.05)。术后7 d,EEN组白蛋白及前白蛋白水平高于DEN组,差异有统计学意义(P<0.05),EEN组白细胞、中性粒细胞均低于DEN组,但差异无统计学意义。结论 EEN可有助于改善食管癌手工吻合术后营养状况及肠道功能恢复,并不增加EN相关并发症的发生。Objective To investigate the significance of early enteral nutrition(EEN)compared with delayed enteral nutrition(DEN)in postoperative nutritional status and intestinal function recovery of patients with esophageal cancer after manual anastomosis.Methods A total of 106 patients with esophageal cancer in our hospital were retrospectively analyzed and they were divided into EEN group(n=58)and DEN group(n=48).The perioperative general condition,postoperative intestinal function recovery,incidence of EN complications,nutritional status recovery and infection index recovery were compared between two groups.Results There was no significant difference in perioperative general condition between the two groups,compared with the DEN group,the EEN group had less early postoperative hunger,higher strength recovery score,shorter first exhaust time,higher mental status score and less hospitalization expenses,the difference was significant(P<0.05).7 d after operation,the levels of albumin and prealbumin in EEN group were higher than those in DEN group,the difference was significant(P<0.05).The leukocytes and neutrophils in EEN group were lower than those in DEN group,but the difference was not statistically significant.Conclusion EEN can improve the nutritional status and intestinal function recovery after manual anastomosis of esophageal cancer,and does not increase the incidence of EN-related complications.
