摘　　要：目的 HPV、TCT联合阴道镜活检在宫颈癌分层筛查中的临床应用价值。方法选取2019年2月至2019年7月新疆阿拉尔市3 000例宫颈癌筛查女性为研究对象,对所有筛查人员进行人乳头状瘤病毒(HPV)检测,仅300例HPV阳性进行宫颈液基薄层细胞学检测(TCT)及阴道镜活检的分层筛查检测,分析HPV与宫颈病变的关系;观察分层筛查检测的灵敏度、特异度、漏诊率、误诊率、准确率。结果 HPV(+)子宫病变占比77.99%,高危HPV(16、18型)子宫病变占比38.00%;TCT分层检测宫颈病变224例,灵敏度、特异度、漏诊率、误诊率、符合率分别为95.73%、77.27%、4.27%、15.15%、91.67%。结论 HPV感染与宫颈癌的发生发展具有一定联系,尤其HPV高危人群需要密切随诊,分层筛查为HPV感染人群提供早发现、早诊断、早治疗的临床应用方法,在临床应用中具有一定的价值意义。Objective Clinical application value of HPV and TCT combined with colposcopy biopsy in stratified screening of cervical cancer.Methods 3000 cervical cancer screening women in Alar,Xinjiang,from February 2019 to July 2019 were selected as study subjects.All screening personnel were tested for human papillomavirus(HPV),while only 300 patients with positive HPV were tested for cervical liquid base thin layer cytology(TCT)and vaginal biopsy stratified screening to analyze the relationship between HPV and cervical lesions.The sensitivity,specificity,missed diagnosis rate,misdiagnosis rate and accuracy of stratified screening were observed.Results HPV(+)uterine lesions accounted for 77.99%,highrisk HPV(type 16,18)uterine lesions accounted for 38.00%;TCT stratified detection of cervical lesions in 224 cases,the sensitivity,specificity,missed diagnosis rate,misdiagnosis rate,coincidence race were 95.73%,77.27%,4.27%,15.15%,91.67%,respectively.Conclusion HPV infection in the occurrence and development of cervical cancer has certain relationship,especially HPV high-risk population needs close follow-up,stratified screening for HPV infected population provides early detection,early diagnosis,early treatment of better clinical application method,in clinical application has certain value.
