血浆Hcy、ALP、IL-17在老年缺血性脑卒中患者中的表达及与颅内血管病变的关系

摘　　要：目的探讨血浆同型半胱氨酸(Hcy)、碱性磷酸酶(ALP)、白细胞介素-17(IL-17)在老年缺血性脑卒中(CIS)患者中的表达及其与颅内血管病变的关系。方法选取2017年6月至2019年7月老年CIS患者92例为观察组,同期92例健康体检者为对照组。比较2组血浆Hcy、ALP、IL-17水平,分析不同血管病变支数、狭窄程度血浆Hcy、ALP、IL-17水平变化及相关性,并探讨CIS发病影响因素及预测价值。结果观察组入院时血浆Hcy、ALP、IL-17水平高于对照组(P<0.05);血浆Hcy、ALP、IL-17水平异常为CIS发病危险因素,随血管病变支数增加、血管狭窄程度加重,血浆Hcy、ALP、IL-17水平呈升高趋势(P<0.05);经Spearman分析,血浆Hcy、ALP、IL-17水平与血管病变支数、狭窄程度呈正相关(r值分别为0.695、0.701、0.581、0.578、0.613、0.543,P<0.05);Hcy、ALP、IL-17预测血管多支病变AUC值分别为0.832、0.734、0.764,Hcy、IL-17预测血管重度狭窄或闭塞AUC值分别为0.925、0.710。结论老年CIS患者血浆Hcy、ALP、IL-17水平异常,与其颅内血管病变程度关系密切,可作为血管病变支数、狭窄程度的预测因子。Objective To investigate the expression of plasma homocysteine(Hcy),alkaline phosphatase(ALP)and interleukin-17(IL-17)in elderly patients with cerebral ischemic stroke(CIS),and to explore their correlation with intracranial vascular lesions.Methods A total of 92 elderly patients with CIS who were treated in our hospital from June 2017 to July 2019 were enrolled as observation group,at the same time,the other 92 healthy subjects were enrolled as control group.The levels of plasma Hcy,ALP and IL-17 were observed and compared between the two groups.Moreover the changes and correlation of plasma levels of Hcy,ALP and IL-17 in different vascular lesions and stenosis degree were analyzed.The influencing factors on CIS and predictive value of these indexes were investigated.Results The plasma levels of Hcy,ALP and IL-17 on admission in observation group were significantly higher than those in control group(P<0.05).The abnormal levels of plasma Hcy,ALP and IL-17 were risk factors for CIS,with the increase of vascular lesions and the degree of vascular stenosis,the plasma levels of Hcy,ALP and IL-17 were increased(P<0.05).The plasma levels of Hcy,ALP and IL-17 were positively correlated with the number of vascular lesions and stenosis degree by Spearman analysis(P<0.05).The AUC values of Hcy,ALP,and IL-17 in predicting multivessel disease were 0.832,0.734,and 0.764,respectively,and the AUC values of Hcy and IL-17 in predicting the severity of stenosis or occlusion were 0.925 and 0.710,respectively.Conclusion The plasma levels of Hcy,ALP and IL-17 in elderly patients with CIS are abnormal,which are closely related to the severity of intracranial vascular disease,which can be regarded as the indexes in predicting the severity of vascular lesion and stenosis.
