贝那普利氢氯噻嗪片对高血压伴阵发性房颤患者房性心律失常再发和心率变异性的影响

摘　　要：目的研究贝那普利氢氯噻嗪片对高血压合并阵发性房颤患者的房性心律失常再发和心率变异性的影响。方法收录2016年1月至2017年1月收治的高血压合并阵发性房颤患者120例,随机分为2组,每组60例。观察组在基础治疗的情况下加用小剂量贝那普利氢氯噻嗪片,对照组单纯给予基础治疗,应用24小时动态心电图,记录用用药前、药后1周、1个月、3个月、6个月、12个月的随访患者的时域参数和频域参数,以及12个月内有无房性心律失常发作和房颤复发情况出现,比较观察组与对照组各项指标是否有差异。结果在12个月的随访中观察组在应用贝那普利氢氯噻嗪片后时域参数及频域参数(SDNN、RMSSD、LF、HF、LF/HF)发生显著性变化(P<0.05)。在随访中高血压并阵发性房颤患者中有26例出现房性心律失常,其中8例在观察组中,18例在对照组中,观察组出现房性心律失常比率中明显小于对照组(P<0.05)。再发房颤的患者观察组5例,对照组8例,差异无统计学意义(P>0.05)。结论高血压合并阵发性房颤患者应用贝那普利氢氯噻嗪片治疗可以减少房性心律失常出现,并可以改善心率变异性。Objective To investigate the effects benazepril/hydrochlorothiazide tablets on the recurrent atrial arrhythmia and heart rate variability in patients with hypertension complicated by paroxysmal atrial fibrillation.Methods A total of 120 patients with hypertension complicated by paroxysmal atrial fibrillation from January 2016 to January 2017 were enrolled in the study,who were randomly divided into control group and observation group,with 60 cases in each group.The patients in control group received basic therapy,however,the patients in observation group,on the basis of control group,were treated by benazepril/hydrochlorothiazide tablets.The heart rate variability and frequency parameter,atrial arrhythmia and fibrillation were observed by 24-h dynamic electrocardiogram(Holter)before medication and at 1w and 1m,3m,6m,12m after treatment.Results During the 12-month follow-up,26 cases of the patients suffered from atrial arrhythmia,of whom,8 cases in observation group and 18 cases in control group,and the incidence rate of atrial arrhythmia in observation group was significantly lower than that in control group(P<0.05).There were 5 cases of recurrence of atrial fibrillation in observation group and 8 cases in control group,there was no significant difference between the two groups(P>0.05).Moreover the changes of time and frequency parameters(SDNN,RMSSD,LF,HF,LF/HF)in observation group changed significantly(P<0.05).Conclusion Benazepril/Hydrochlorothiazide tablets can decrease the incidence of atrial arrhythmia and improve heart rate variability in patients with hypertension complicated by paroxysmal atrial fibrillation.
