布南色林联合天智颗粒治疗慢性精神分裂症认知功能障碍的对照研究

摘　　要：〔目的〕探讨布南色林对精神分裂症的治疗效果,比较单用布南色林和联合天智颗粒对认知功能改善的差异。〔方法〕选取某院2018年6月至7月入院病程不少于5 a的精神分裂症患者60例,随机分为布南色林单用组(以下简称单用组)和布南色林联用天智颗粒组(以下简称联用组),每组30例,观察时间为8周。于治疗前、治疗后第4周、第8周分别对单用组、联用组患者采用阳性和阴性症状量表(PANSS)进行精神症状评价,采用韦氏数字广度测验(DS)、威斯康星卡片分类测试(WCST)进行认知功能评价,采用酶联免疫吸附法测定血清中脑源性神经营养因子(BDNF),同时招募30名健康受试者并进行上述评价。〔结果〕治疗前单用组和联用组PANSS各项评分无显著差异(P>0.05),而治疗4周和8周后两组均较治疗前低(P<0.05);单用组在治疗4周和8周后,数字广度测验各项评分均较治疗前有所改善(P<0.05),联用组在治疗8周后各项评分均高于单用组(P<0.05);单用组在4周和8周后,威斯康星分类测试各项评分均有改善,联用组在治疗4周、8周后仅正确应答数高于单用组(P<0.05);治疗8周后血清中BDNF水平联用组高于单用组(P<0.05)。〔结论〕布南色林可有效改善慢性精神分裂患者阳性症状、阴性症状和认知功能,联用天智颗粒可更好地改善认知功能。[Objective]To investigate the therapeutic effect of blonanserin on schizophrenia and compare the improvement of cognitive function between taking blonanserin with and without Tianzhi granules.[Methods] 60 patients with schizophrenia(duration of not less than 5 years) in a hospital from June 2018 to July 2018,who were randomly selected,using random number table method were divided into the single blonanserin group(hereinafter referred to as alone group) and blonanserin combined with Tianzhi granules group(hereinafter referred to as the combination group).30 cases in each group,period of observation for 8 weeks.The severity of the disease was evaluated using the positive and negative symptom scale(PANSS) before the experiment,4 weeks after the treatment,and 8 weeks after the treatment,and the cognitive function was evaluated using the weiss digital breadth test(DS) and the Wisconsin card classification test(WCST).Besides the serum level of BDNF was detected by enzyme-linked immunosorbent analysis.At the same time,30 healthy subjects were recruited and evaluated.[Results]There was no significant difference in PANSS scores between the two groups before and after treatment(P>0.05),but the scores were lower in both groups at 4 weeks and 8 weeks after treatment(P<0.05).After 4 weeks and 8 weeks of treatment,the scores of the digital breadth test were improved compared with those before treatment(P<0.05),and the scores of the combined treatment group were higher than those of the single-use group after 8 weeks of treatment(P<0.05).The Wisconsin classification test scores improved after 4 and 8 weeks in the single-use group,and only the number of correct responses was higher in the combined group after 4 and 8 weeks(P<0.05).After 8 weeks of treatment,serum BDNF level in the combined group was higher than those in the single group.[Conclusion] Blonanserin can effectively improve positive symptoms,negative symptoms and cognitive function in patients with chronic schizophrenia,and tianzhi granule can improve cognitive functi
