TLR4在壶腹部周围癌中的表达及临床意义

摘　　要：〔目的〕探讨壶腹部周围癌中Toll样受体4 (TLR4)的表达水平及其临床意义。〔方法〕选取某医院43例壶腹部周围癌手术患者,采用逆转录聚合酶链反应(RT?PCR)检测TLR4 mRNA表达水平,采用免疫组化染色技术检测TLR4的蛋白表达水平情况,同时收集患者年龄、性别、术前总胆红素水平、肿瘤大小、淋巴结转移、临床分期、分化程度等临床资料。〔结果〕43例配对的壶腹部周围癌患者肿瘤组织中TLR4 mRNA水平明显高于癌旁组织,且二者差异具有统计学意义(P<0.05)。43例癌组织中TLR4蛋白表达的阳性率为58.1%,TLR4的表达在年龄、性别、术前胆红素水平、肿瘤大小等方面差异无统计学意义(P>0.05),而不同临床分期、肿瘤分化程度以及淋巴结是否转移、患者的TLR4蛋白表达差异有统计学意义(P<0.05)。〔结论〕壶腹部周围癌患者TLR4高表达,对于判断预后和指导治疗具有一定的临床价值。[Objective]The purpose of this study was to investigate the expression and clinical significance of Toll-Iike receptor4(TLR4)in patients with periampullary cancer.[Methods]In this study,43 patients with periampullary cancer in a hospital wereselected.RT-PCR was used to detect the levels of TLR4 mRNA,and immunohistochemical SP method was used to detect theexpressions of TLR4 protein,and whose age,gender,preoperative serum bilirubin levels,tumor size,lymph node metastasis,TNM stage and differentiation were also collected.[Resultsi]The TLR4 mRNA relative expression in periampullary cancer tissuewas significantly higher than that in paracarcinomous tssue(P<0.05),The positive expression rates of' TLR4 protein inperiampullary cancer lesion were 58.1%,The expression of TLR4 protein was no significant difference among gender,age,preoperative serum bilirubin levels,tumor size(P>0.05),histology except lymph node metastasis,TNM stage and differentiation(P<0.05).[Conclusion]The higher expression of TLR4 play an important role in the progression of periampullary cancer.which had certain clinical value in evaluating prognosis in patients with periampullary cancer.
