有晶状体眼后房型人工晶状体植入术治疗近视的有效性和安全性研究

摘　　要：目的探讨有晶状体眼后房型人工晶状体(implantable contact lens,ICL)植入术治疗近视的有效性和安全性。方法回顾性病例研究。收集2020年1月至6月于中日友好医院眼科接受植入前免黏弹剂ICL植入术治疗近视患者共26例51眼。统计并记录所有患者术前及术后1周、1个月、3个月及6个月的裸眼视力(UCVA)、最佳矫正视力(BCVA)、屈光状态、拱高、眼压和手术相关并发症情况。采用重复测量方差分析方法进行统计学分析。结果26例(51眼)患者中术后1周UCVA≤0 logMAR的有46眼(90.2%),术后1个月、3个月均有48眼(94.1%)UCVA≤0 logMAR;手术的有效性指数为1.142±0.176。术后随访终点患者BCVA(-0.042±0.066)logMAR较术前BCVA(0.011±0.045)logMAR显著提高(F=32.609,P=0.000),手术的安全性指数为1.150±0.165。随访终点患者残留等效球镜度在±0.50 D和±1.00 D内分别有38眼(74.5%)、47眼(92.2%);随访终点患者拱高为130~1145(465.85±238.23)μm。患者术后眼压与术前相比差异无统计学意义(F=4.338,P=0.285)。所有患者均未出现瞳孔阻滞、前囊膜混浊、白内障、角膜内皮失代偿、眼内炎等严重并发症。结论植入前免黏弹剂、单切口的ICL植入术矫正近视具有良好的有效性、安全性,值得在临床上推广应用。Objective To investigate the efficacy and safety of pre-implantation viscoelastic free single incision phakic posterior chamber implantable collamer lens implantable collamer lens(PPC-ICL)implantation for myopia.Methods A retrospective study was performed on 26 patients(51 eyes)with myopia who underwent ICL implantation without viscoelastic before implantation in the China-Japan Friendship Hospital from January 2020 to August 2020.We measured uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA)and refractive error,vault,intraocular pressure(IOP)preoperatively and postoperatively at 1 week,1 month,3 months and 6 months.The operative and postoperative complications were observed.The results were analyzed with repeated measures analysis of variance.Results Forty six eyes(90.2%)had UCVA≤0 logMAR,and 48 eyes(94.1%)had UCVA≤0 logMAR at 1 month and 3 months after the surgery.The efficacy index was 1.142±0.176.The BCVA at the end of observation was significantly increased than that before the surgery(-0.042±0.066)logMAR vs.(0.011±0.045)logMAR(F=32.609,P=0.000).The safety index was 1.150±0.165.At the end of observation,38 eyes(74.5%)and 47 eyes(92.2%)had residual spherical equivalent within±0.50 D and±1.00 D,respectively;the vault was 130~1145(465.85±238.23)μm.There was no significant difference between preoperative and postoperative IOP(F=4.338,P=0.285).No pupil block,anterior capsule opacity,cataract,decompensation of corneal endothelium,endophthalmitis and other serious complication was found.Conclusion Pre-implantation viscoelastic free single incision PPC-ICL implantation has good safety and efficacy in myopic correction.
