摘　　要：目的比较Vitalium2000与纯钛材料可摘局部义齿修复效果及对基牙牙周的影响。方法收集2017年6月至2019年6月于我科进行治疗的89例牙列缺损患者作为观察对象,所有患者均给予可摘局部义齿修复治疗。回顾性分析患者临床资料,按照材料不同分为Vitalium2000组(n=46)和纯钛组(n=43)。Vitalium2000组患者的义齿支架材料选用Vitalium2000材料,纯钛组患者的义齿支架材料选用纯钛材料。随访1年,比较两组患者咀嚼效率、就位率、自我评价满意度(包括轻巧舒适度、美观度和异味度满意度)、基牙菌斑指数、探诊深度和牙周病发生率。结果随访1年后,与纯钛组相比,Vitalium2000组患者咀嚼效率和就位率提高[(89.71±4.72)%vs.(78.43±4.16)%,(95.65%vs.81.40%)],评价满意度中的轻巧舒适度、美观度和异味度满意度提高[(95.65%vs.79.07%),(97.83%vs.74.42%),(93.48%vs.76.74%)],基牙菌斑指数和探诊深度降低[(2.30±0.70)vs.(2.81±0.82),(1.81±0.50)mm vs.(2.59±0.76)mm)],牙周病发生率降低(4.35%vs.23.25%),组间比较差异均有统计学意义(P<0.05)。结论与纯钛材料相比,Vitalium2000材料作为可摘局部义齿支架材料修复后具有更高的咀嚼效率、就位率和自我评价满意度,且对基牙牙周具有一定保护作用,能够减少牙周病的发生。Objective To compare the restoration effect of removable partial denture made of Vitalium2000 and pure titanium and their influence on abutment periodontium.Methods A total of 89 patients with dentition defects treated in our department from June 2017 to June 2019 were collected as the objects,and all patients were treated with removable partial denture.Clinical data of patients were retrospectively analyzed and divided into the Vitalium2000 group(n=46)and the pure titanium group(n=43).Vitalium2000 was used as denture frame material for patients in the Vitalium2000 group,and the pure titanium was used as denture frame material for patients in pure titanium group.Patients were followed up for one year.The masticatory efficiency,the insertion rate,the self-evaluation satisfaction(including lightness,comfort,beauty and odor),plaque index,probing depth and periodontal disease incidence were compared between the two groups.Results After one year follow-up,compared with the pure titanium group,the masticatory efficiency and the insertion rate in Vitalium2000 group increased([89.71±4.72]%vs.[78.43±4.16]%,[95.65%vs.81.40%]).The lightness,comfort,aesthetics and odor in patients′satisfaction increased([95.65%vs.79.07%],[97.83%vs.74.42%],[93.48%vs.76.74%]).The base plaque index and probing depth decreased([2.30±0.70]vs.[2.81±0.82],[1.81±0.50]mm vs.[2.59±0.76]mm),and the incidence of periodontal disease decreased(4.35%vs.23.25%),with statistical differences between groups(P<0.05).Conclusion Compared with pure titanium material,Vitalium2000 material has higher masticatory efficiency,insertion rate and self-evaluation satisfaction after being used as removable partial denture frame material,which has certain protective effect on abutment periodontium and can reduce the occurrence of periodontal disease.
