摘  要:目的比较3种不同的固定技术对颅内肿瘤射波刀(CK)治疗过程中位置误差的影响。方法收集20例行CK治疗的颅内肿瘤患者,按照不同体位固定技术随机分为3组:采用头颈肩板+头颈肩热塑膜固定的患者为头颈肩组(7例);采用头颈肩板+U型热塑头膜固定的患者为U型头膜组(7例);采用头板+热塑头膜固定的患者为小头膜组(6例)。比较3种不同固定技术对治疗过程中位置误差的影响。结果本研究取得治疗过程中的实际体位位移误差即相对误差。方差分析结果显示,3组间的相对误差差别均有统计学意义(P<0.05);SNK法两两比较结果显示,小头模组的相对误差大于U型头模和头颈肩组,而在drX(L-R旋转)方向上小头模组和头颈肩组均大于U型头模组(P<0.05)。结论3种固定方式中,小头模组的固定方式相对误差最大,而U型头模组固定效果最优。推荐采用头颈肩板+U型热塑头膜作为颅内肿瘤CK放射治疗的固定方式。Objective To compare the effect of three different fixation techniques on the position error in the treatment of intracranial tumor with the cyberknife(CK).Methods Twenty patients with intracranial tumor treated by cyberknife were randomly selected and randomly divided into three groups according to different postural fixation techniques:head and neck shoulder plate+head and neck shoulder thermoplastic film fixation group(7 cases);Patients who were fixed with head and neck shoulder plate+U-shaped thermoplastic head membrane were in U-shaped head membrane group(7 cases);Patients fixed with head plate and thermoplastic head membrane were small head membrane group(6 cases).The influence of three different fixation techniques on the position error in the treatment process was compared and analyzed.Results In this study,the actual displacement error of body position in the course of treatment was obtained,that is,the relative error|d|data.ANOVA was used,and the differences among the three groups were statistically significant.The error of small head module was greater than that of U-head module and head neck shoulder group,while the error of small head module and head neck shoulder group were greater than that of U-head module in the direction of drX(left-right revolve).Conclusion Among the three fixation methods,the small head module has the largest error,while the head-neck-shoulder group and the U-head module have the smallest error and little difference.The fixation effect of the U-head module is optimal.Therefore,head and neck shoulder plate+U-shaped module is recommended to be used as the fixation method for patients undergoing intracranial tumor radiosurgery.
