摘 要:目的分析血管紧张素Ⅱ(AngⅡ)水平在肝硬化患者诊断中的价值及与终末期肝病模型评分(MELD评分)的相关性。方法入选肝硬化患者87例为试验组,另选同期82名年龄、性别与其匹配的健康体检者为对照组。测定各组对象间肝功能及血清AngⅡ的水平。结果肝硬化组病人血清ALT、AST、TBil、AngⅡ水平较高,ALB水平较低,与对照组比较差异均有统计学意义(P<0.05)。AngⅡ水平与ALT、AST、TBil呈正相关,与ALB呈负相关。肝硬化患者血清AngⅡ水平在不同Child-Pugh分级组间差异有统计学意义(P<0.05)。MELD不同分值间AngⅡ水平差异有统计学意义(P<0.05)。Pearson相关分析发现,血清AngⅡ水平与MELD评分存在较好的正相关关系(r=0.459,P=0.000)。肝硬化合并腹水患者血清AngⅡ水平明显高于肝硬化未合并腹水患者(P<0.01)。结论随着肝硬化病情进展,血管紧张素Ⅱ的水平不断升高,与肝功能指标及MELD评分存在正相关性。Objective Objective:To analyze the value of angiotensinⅡ(AngⅡ)in the diagnosis of liver cirrhosis and its correlation with MELD score.Methods 87 patients with liver cirrhosis were selected as the experimental group,82 healthy people with age and gender matched in the same period were selected as the control group.Liver function and serum AngⅡlevels were measured.Results The levels of ALT,AST,TBIL and AngⅡin patients with liver cirrhosis were higher than those in the control group(P<0.05).AngⅡlevel was correlated with ALT,AST and TBIL,but negatively correlated with ALB.There was significant difference in serum angⅡlevel among different child Pugh grades(P<0.05).There was significant difference in AngⅡlevel among different MELD scores(P<0.05),Correlation analysis showed that there was a good positive correlation between serum AngⅡlevel and MELD score(r=0.459,P=0.000).The level of AngⅡin patients with cirrhosis complicated with ascites was significantly higher than that in patients without ascites(P<0.01).Conclusion With the progress of liver cirrhosis,the level of AngⅡis increasing,and there is a positive correlation between AngⅡand liver function and MELD score.
