双歧杆菌四联活菌片联合早期肠内营养对SAP患者炎性因子水平及肠道菌群的影响

摘　　要：目的观察双歧杆菌四联活菌片联合早期肠内营养对重症急性胰腺炎(SAP)患者炎性因子水平及肠道菌群的影响。方法选择2018年3月-2019年11月四川省中江县人民医院收治的SAP患者84例,采用抓阄法随机分为观察组和对照组各42例。对照组接受早期肠内营养,观察组采用双歧杆菌四联活菌片联合早期肠内营养。比较2组患者治疗效果及治疗前后炎性因子水平、肠道菌群情况。结果观察组治疗总有效率为95.2%,高于对照组81.0%(χ^(2)=4.087,P=0.043);治疗后,2组肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)及C反应蛋白(CRP)均低于治疗前,且观察组治疗后低于对照组(P<0.01);治疗后,2组双歧杆菌、乳酸杆菌比例均高于治疗前,大肠杆菌、葡萄球菌比例均低于治疗前,且观察组变化幅度大于对照组(P<0.01)。结论双歧杆菌四联活菌片联合早期肠内营养用于SAP的效果显著,可控制患者炎性因子水平,调节菌群平衡,防止菌群移位发生。Objective To investigate the effects of bifidobacterium quadruple live bacteria tablets combined with early enteral nutrition on the levels of inflammatory factors and intestinal flora in patients with severe acute pancreatitis(SAP).Methods A total of 84 SAP patients admitted to the People’s Hospital of Zhongjiang County,Sichuan Province from March2018 to November 2019 were selected and randomly divided into observation group and control group with 42 cases by lottery method.The control group received early enteral nutrition,and the observation group received bifidobacterium quadruple viable tablets combined with early enteral nutrition.The treatment effect,inflammatory factor levels and intestinal flora before and after treatment were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.2%,which was higher than 81.0%in the control group(χ^(2)=4.087,P=0.043);After treatment,the tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)of the two groups were lower than before treatment,and the observation group were lower than the control group after treatment(P<0.01);After treatment,the ratios of bifidobacteria and lactobacilli in the two groups were higher than those before treatment,and the ratios of Escherichia coli and Staphylococcus were lower than before treatment,and the observation group changed more than the control group(P<0.01).Conclusion The bifidobacterium quadruple live bacteria tablet combined with early enteral nutrition has a satisfactory effect in SAP.It can control the level of inflammatory factors in patients,can also regulate the flora balance,and prevent the occurrence of flora shift.
