不同超前镇痛对胸腔镜肺癌根治术后患者镇痛效果及血流动力学参数的影响

摘　　要：目的研究不同超前镇痛对胸腔镜肺癌根治术后患者镇痛效果及血流动力学参数的影响。方法选取本院2019年10月至2020年9月收治的80例肺癌患者,根据简单随机化法将其分为对照组和观察组,每组40例。两组患者均接受胸腔镜肺癌根治术治疗。对照组应用地塞米松进行超前镇痛,观察组应用帕瑞昔布钠进行超前镇痛。比较两组的镇痛效果、血流动力学参数及血清炎症因子水平。结果观察组术后1、6、12、24 h的NRS评分均显著低于对照组(P<0.05)。观察组术后24 h的平均动脉压、心率、IL-6、IL-10及TNF-α水平均低于对照组(P<0.05)。结论同地塞米松相比,在胸腔镜肺癌根治术前应用帕瑞昔布钠进行超前镇痛可达到良好的术后镇痛效果,能够稳定血流动力学,且不会加重炎症反应。Objective To study the effects of different preemptive analgesia on postoperative analgesia and hemodynamic parameters in patients undergoing thoracoscopic radical resection of lung cancer.Methods Eighty patients with lung cancer enrolled in our hospital from October 2019 to September 2020 were selected and divided into control group and observation group according to simple randomization method,with 40 cases in each group.Both groups were treated with thoracoscopic radical resection of lung cancer.The control group was given dexamethasone for preemptive analgesia,and the observation group was given parecoxib sodium for preemptive analgesia.The analgesic effects,hemodynamic parameters and serum inflammatory factors levels were compared between the two groups.Results The NRS scores at 1,6,12 and 24 h after operation in the observation group were significantly lower than those in the control group(P<0.05).The mean arterial pressure,heart rate,levels of IL-6,IL-10 and TNF-α at 24 h after operation in the observation group were lower than those in the control group(P<0.05).Conclusion Compared with dexamethasone,preemptive analgesia with parecoxib sodium for thoracoscopic radical resection of lung cancer has good postoperative analgesic effect,stable hemodynamics,and does not aggravate the inflammatory reaction.
