改良引导骨再生技术在后牙区种植患者中的应用及牙槽嵴骨量变化与种植体周围软组织变化的观察

摘　　要：目的研究改良引导骨再生技术在后牙区种植患者中应用,并观察牙槽嵴骨量变化、种植体周围软组织变化。方法选择行后牙区种植术患者73例为研究对象,随机分为治疗组38例,对照组35例,治疗组给予改良引导骨再生技术,对照组应用引导骨再生技术。采用CBCT分别于种植体术后即刻、术后半年进行拍摄,测量种植体牙槽骨骨量变化与采用改良出血指数(mSBI)、探诊深度及改良菌斑指数(mPI)评价种植体周围软组织变化。结果2组患者各位点牙槽嵴骨量变化差异无统计学意义(P>0.05);2组患者mSBI、探诊深度水平差异无统计学意义(P>0.05),治疗组mPI评分小于对照组(P<0.05);随访半年后,所有患者种植体无严重不良反应,对照组有1例患者术后有轻微肿胀,经治疗后完全恢复,均良好骨结合,存留率均为100%;治疗组患者满意度高于对照组(P<0.05)。结论改良引导骨再生技术在后牙区种植患者中应用对牙槽嵴骨量变化及种植周围体软组织变化与引导骨再生技术几乎相同,但改良引导骨再生技术若不使用可吸收膜骨增量效果可能更佳,且无严重不良反应,患者满意度较高。Objective To investigate the application of modified guided bone regeneration in patients with posterior dental implant restoration and to observe the changes of alveolar ridge bone mass and soft tissue around the implant.Methods 73 patients who underwent posterior dental implant restoration were enrolled and divided into two groups by random number table methods.The treatment group(n=38)was treated with the modified guided bone regeneration technique and the control group with the guided bone regeneration.Bone mineral density(CBCT)was used to measure the bone mass of alveolar bone,and the modified sulcus bleeding index(mSBI),probing depth and modified plaque index(mPI)were used to evaluate the peri-implant soft tissue changes.Results There was no significant difference in alveolar ridge bone mass between the two groups(P>0.05).mSBI and probing depth had no significant difference between the two groups(P>0.05),while the mPI score in treatment groupwas lower than that in control group(P<0.05).There was no serious adverse reactions in all patients,but one patient in the control group showed slight postoperative swelling and recovered completely after treatment.The retention rate of implants was 100%.The satisfaction of patient in treatment group was higher than that in control group(P<0.05).Conclusion The changes of alveolar ridge bone mass and peri-implant soft tissue of the modified guided bone regeneration in the patients with posterior dental implant restoration is almost the same as that of the guided bone regeneration.However,the absence of absorbable membrane in the modified guided bone regeneration may be better for alveolar ridge augmentation.There are no serious adverse reactions,and the patient satisfaction is higher.
