胃石症48例的临床特征及有效治疗方法的选择

摘　　要：目的探讨胃石症的特征及相应的经济且有效的治疗方法。方法回顾性分析2011—2021年首都医科大学附属北京安贞医院收治的胃石症患者48例的临床资料及治疗方法。结果60岁以上者32例(66.7%);胃镜见胃石的位置在胃内者47例(97.7%);胃石直径2.5~12cm。植物性胃石47例(97.9%),药物性胃石1例(2.1%);进食山楂/柿子者38例(79.2%),进食黑枣者7例(14.6%),牙模、大量高纤维食物导致3例(6.3%)。口服碳酸氢钠溶液和碳酸饮料5d后,82.6%患者溶石成功;溶石失败患者行内镜下碎石术,采用手术治疗者3例。结论胃石症以植物性胃石为主,胃石与消化性溃疡、反流性食管炎密切相关。山楂胃石和柿石症患者采用口服可乐和/或碳酸氢钠液体溶石疗法有效率高,而黑枣胃石症采取口服液体疗法效果差,可以内镜下碎石治疗或外科手术治疗。Objective To explore the characteristics of gastrolithiasis and the corresponding economic and effective treatment.Method the clinical data and treatment of 48 patients with gastrolithiasis admitted to Beijing Anzhen Hospital Affiliated to Capital Medical University from 2011 to 2021 were retrospectively analyzed.Result 32 cases(66.7%)were over 60 years old,47 cases(97.7%)were located in the stomach by gastroscope,and the diameter of gastrolith was 2.5cm-12cm.There were 47 cases(97.9%)of plant gastrolith,1 case(2.1%)of drug gastrolith,38 cases(79.2%)of Hawthorn/persimmon,7 cases(14.6%)of black jujube,and 3 cases(6.3%)of dental mold and high fiber food.After oral administration of sodium bicarbonate liquor and carbonated drinks for 5 days,82.6%of the patients were successful in Litholysis;3 patients failed in Litholysis underwent endoscopic lithotripsy and were treated with surgery.Conclusion the main cause of gastrolithiasis is plant gastrolithiasis,which is closely related to peptic ulcer and reflux esophagitis.Oral liquid Litholysis is effective,economical and well tolerated in patients with hawthorn and persimmon stones.However,The effect of oral liquid therapy on gastrolithiasis of black jujube is poor,which can be combined with endoscopic lithotripsy or surgical operation.
