无水乙醇联合碘油及弹簧圈治疗巨大肾血管平滑肌脂肪瘤中期疗效及安全性

摘　　要：目的探讨无水乙醇联合碘油及弹簧圈治疗巨大肾血管平滑肌脂肪瘤(AML)中期疗效和安全性.方法回顾性选取行无水乙醇碘油混合液联合弹簧圈治疗巨大肾AML患者共7例,分析介入栓塞手术成功率,并比较栓塞前后瘤体大小、病灶缓解率及并发症发生率,观察患者术前和术后3 d及1月肾功能变化.结果7例患者共7个病灶顺利完成手术,手术成功率100%.与术前相比,术后1月及术后6月瘤体体积差异有统计学意义(P<0.05),术后3 d及术后1月肾功能变化差异无统计学意义(P>0.05),术后无严重不良反应及肿瘤复发.结论无水乙醇碘油混合液联合弹簧圈治疗巨大肾AML安全有效,价格低廉,值得临床推广应用.Objective To investigate the mid-term efficacy and safety of anhydrous ethanol and lipiodol mixture combined with coil in the treatment of giant renal angiomyolipoma(AML).Methods The clinical data of 7 patients with giant renal AML and treated with anhydrous ethanol and lipiodol mixture combined with coil were reviewed.The success rate of interventional embolization operation,the size of tumors before and after embolization,remission rate and postoperative complications were analyzed,and the renal function were observed before,after 3 days and 1 month.Results Seven patients with 7 lesions were successully completed the operation with a 100%success rate.Compared with preoperative,the differences in the tumor volume between preoperative and postoperative 1 month and 6 months were statistically significant(P<0.05);the differences in the renal function between preoperative and postoperative 3 days and 1 month were not statistically significant(P>0.05).There were no serious complications and recurrence after operation.Conclusion The anhydrous ethanol and lipiodol mixture combined with coil is safe,effective,and inexpensive for the treatment of giant renal AML,which is worthy of clinical application.
