摘　　要：目的探讨外周血炎症指标对乳腺癌新辅助化疗(NAC)疗效的预测价值。方法回顾性分析2013年1月至2019年12月187例在本院行NAC患者资料,采用单因素和多因素Logistic回归分析外周血炎症指标(NLR、PLR、LMR、SII)和NAC术后病理MP分级的关系。结果单因素结果表明NAC疗效与ER、PR、Her-2、NLR、PLR、SII有关(P<0.05),在117例患者中,单因素结果表明NAC疗效与Her-2、NLR有关(P<0.05),Logistic回归显示PR、Her-2、NLR(正相关)是NAC疗效的独立预测因素,PR、Her-2、NLR(负相关)是NAC疗效的独立预测因素,AUC值为0.759。结论在HR-、Her2+、Ki-67高表达(>30%)的乳腺癌患者中,NLR可做为PR、Her-2等病理指标的补充,可预测乳腺癌NAC疗效。Objective To investigate the predictive value of peripheral inflammatory index in neoadjuvant chemotherapy(NAC) for breast cancer. Methods The data of 187 patients with NAC from January 2013 to December 2019 in our hospital were retrospectively analyzed. Univariate and multivariate logistic regression was used to analyze the relationship between peripheral blood inflammatory indexes(NLR, PLR, LMR, SII) and pathological MP grading after neoadjuvant chemotherapy. Results Univariate analysis showed that the efficacy of NAC was related to Er, PR, HER-2, NLR, PLR and SII(P<0.05). In 117 patients, the univariate results showed that NAC efficacy was related to HER-2 and NLR( P<0.05). Logistic regression showed that PR, HER-2 and NLR(positive correlation) were independent predictors of NAC efficacy, while PR, HER-2 and NLR(negative correlation) were independent predictors of NAC efficacy, with AUC value of 0.759. Conclusion In breast cancer patients with high expression of HR-, HER2 +, Ki-67(>30%), NLR can be used as a supplement of PR, HER2 and other pathological indicators, which can predict the efficacy of neoadjuvant chemotherapy for breast cancer.
