摘  要:目的:根据疾病诊断相关分组(DRG)对2018年某医院的内科科室进行绩效评价,为持续改进医疗服务水平、促进医院精细化管理提供依据。方法:运用综合指数法和秩和比法(RSR),从服务能力、服务效率和医疗安全三个维度对20个内科科室进行绩效评价。结果:综合指数位列前三的科室分别是呼吸内科一、心内科一和肿瘤一科;排名倒数的三个科室分别是康复科、老年医学科和神经内科一;采用RSR法将不同科室的医疗服务水平划分为好、中、差3档,呼吸内科一、心内科二、心内科一和肿瘤一科被归为"好",康复科、神经内科一和老年医学科被归为"差",其余13个科室被归为"中"。结论:不同科室在能力、效率和安全维度上存在着不同程度的缺陷,科室应根据自身的薄弱环节进行有针对性的改进,以提高整体医疗服务水平。Objective:Through diagnosis-related group(DRG)to evaluate the internal medical service performance in a certainhospital,toprovide a basis for continuously improving the medical service and promoting the refined management of hospital.Methods:Using the synthetic index method and theRank-Sum-Ratio(RSR)method to evaluate the performance of 20 medical departments from three aspects,namely service capability,service efficiency and medical safety.Results:The top three departments whose composite index values were best were Department of RespirationⅠ,Department of CardiologyⅠ,and Department of Medical OncologyⅠ.The last three departments were Department of Rehabilitation,Department of Geriatrics and Department of NeurolgyⅠ.Three grades of the medical service performance,namely Good,Average and Poor,were divided by the RSR method.Department of RespirationⅠ,Department of CardiologyⅡ,Department of CardiologyⅠ,and Department of Medical OncologyⅠwere divided into Good.Department of Rehabilitation,Department of NeurolgyⅠand Department of Geriatrics were divided into Poor.The other departments were divided into Average.Conclusions:Defects of different degrees of the departments were observed in the service capability,efficiency and safety.Based on one’s own weakness,targeted improvements should be taken in order to improve the overall medical service level.
