甘草泻心汤加味治疗寒热错杂型复发性口腔溃疡疗效及机制研究

摘　　要：目的观察甘草泻心汤加味辅助治疗寒热错杂型复发性口腔溃疡的效果,探讨其治疗作用机制。方法选择2018年7月—2019年7月在河北省沧州中西医结合医院治疗的240例复发性口腔溃疡患者,随机分为2组各120例,对照组给予常规西医综合治疗,观察组在此基础上联合甘草泻心汤加味治疗,连续治疗14 d。评估2组短期治疗效果,观察2组中医症状积分、唾液表皮生长因子(EGF)水平及血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)]变化情况,随访2组复发情况。结果观察组总有效率为94.2%(113/120),高于对照组的78.3%(94/120),差异有统计学意义(P<0.05)。2组治疗后中医症状积分、唾液EGF水平及血清TNF-α、IL-6、IL-8、IL-10水平均明显降低(P均<0.05),且观察组治疗后的中医症状积分及各项检测指标水平均明显低于对照组(P均<0.05)。观察组随访3,6,9,12个月的病情复发率均明显低于同期对照组(P均<0.05)。结论甘草泻心汤加味辅助治疗寒热错杂型复发性口腔溃疡患者效果优于单纯西医治疗,有助于改善中医症状,促进损伤黏膜修复,预防病情复发,可能与减轻炎性反应有关。Objective It is to observe the effect of adjuvant treatment with Gancao Xiexin Decoction on recurrent oral ulcers of cold-heat complex type,and to explore its therapeutic mechanism.Methods A total of 240 patients with recurrent oral ulcers who were treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from July 2018 to July 2019 were randomly divided into two groups with 120 cases in each group.The control group was given conventional comprehensive treatment with western medicine,and the observation group was treated with Gancao Xiexin Decoction based on the control group,both groups were treated for 14 days.The short-term treatment effects of the 2 groups were evaluated,and the changes in the TCM symptom score,the levels of saliva epidermal growth factor(EGF)and serum inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10)]were observed,the recurrence of the 2 groups were followed up.Results The total effective rate of the observation group was 94.2%(113/120),which was higher than 78.3%(94/120)of the control group,and the difference was statistically significant(P<0.05).The TCM symptom scores,levels of saliva EGF and serum TNF-α,IL-6,IL-8,IL-10 were significantly reduced after treatment in the two groups(all P<0.05),and the TCM symptom scores and each detecting indicators of the observation group were significantly lower than those of the control group after treatment(all P<0.05).The recurrence rate of the observation group was significantly lower than that of the control group during the follow-up period of 3,6,9,and 12 months(all P<0.05).Conclusion Gancao Xiexin Decoction is more effective than simple western medicine in the treatment of patients with recurrent oral ulcers of cold-heat complex type,it is helpful for improving the symptoms of traditional Chinese medicine,promoting the repair of damaged mucosa,preventing the recurrence of the disease,and the mechanism may be related to reduce the inflammatory reaction.
