摘  要:目的观察高压氧联合虚拟现实技术对脑梗死伴偏瘫患者运动协调能力的影响。方法将120例脑梗死伴偏瘫患者随机分别为高压氧组、虚拟现实技术组及综合治疗组,每组40例。高压氧组行高压氧治疗,虚拟技术组行虚拟现实技术治疗,综合治疗组行高压氧联合虚拟现实技术治疗。治疗前、治疗4周后采用改良Barthel指数(mBI)、Fugl-Meyer下肢功能评定(FMA-L)评分、Berg平衡量表(BBS)及站起-行走计时测试(TUGT)分别对3组患者的日常生活能力、下肢运动功能平衡状态、下肢运动协调能力进行评定。结果治疗前,3组患者的mBI评分、FMA-L评分、BBS评分及TUGT比较,差异均无统计学意义(均P>0.05)。治疗4周后,3组患者的mBI评分、FMA-L评分及BBS评分均高于治疗前,TUGT均低于治疗前(均P<0.05);3组患者的mBI评分、FMA-L评分及BBS评分由高到低均为综合治疗组>虚拟技术组>高压氧组,TUGT由低到高为综合治疗组>虚拟技术组>高压氧组(均P<0.05)。结论高压氧与虚拟现实技术对脑梗死伴偏瘫患者的康复效果具有协同增效作用,两者联合能进一步提高患者下肢运动协调功能、步态协调平衡及日常生活能力。Objective To observe the effect of hyperbaric oxygen combined with virtual reality technology on motor coordination ability in cerebral infarction patients complicated with unilateral hemiplegia.Methods A total of 120 cases of cerebral infarction complicated with unilateral hemiplegia were randomly divided into hyperbaric oxygen group,virtual reality technology group and comprehensive treatment group,with 40 cases in each group.The hyperbaric oxygen group was given hyperbaric oxygen treatment,the virtual reality technology group was given virtual reality technology treatment,and the comprehensive treatment group was given hyperbaric oxygen combined with virtual reality technology therapy.The modified Barthel index(mBI),the Fugl-Meyer Assessment of Lower Extremity(FMA-L)score,the Berg Balance Function Scale(BBS)and the Timed Up and Go Test(TUGT)were used to assess ability of daily living,motor function and equilibrium state of lower extremity,and motor coordination ability of lower extremity of patients in the three groups before and after four weeks of treatment.Results Before treatment,there was no statistically significant difference in mBI score,FMA-L score,BBS score or TUGT among the three groups(all P>0.05).Four weeks after treatment,mBI score,FMA-L score and BBS score were higher and TUGT was lower in the three groups than those before treatment(all P<0.05);mBI score,FMA-L score and BBS score decrease and TUGT increased in the sequence of the comprehensive treatment group,the virtual reality technology group and the hyperbaric oxygen group(all P<0.05).Conclusion Hyperbaric oxygen combined with virtual reality technology has synergistic effect on the rehabilitation efficacy for cerebral infarction patients complicated with unilateral hemiplegia,and their combination can further improve patients′motor coordination function of lower extremity,gait coordination and balance,and ability of daily living.
