腭裂术后言语康复治疗对嗓音影响的主观评价

摘　　要：目的探讨腭裂修补术后语音训练前、后患儿嗓音声学参数GRBAS的听主观评估后临床变化及语音清晰度的临床变化。方法对40例腭裂修补术后患儿进行包括构音和嗓音的言语康复治疗,并对治疗前、后进行长音/a/的GRBAS(总嘶哑度G、粗糙度R、气息度B、紧张度A、无力度S)评级分值及语音清晰度(%)分析比较。结果言语康复治疗后G、R评级分值及语音清晰度(%)较治疗前有明显改善,差异有统计学意义(P<0.05),治疗前后B、A、S评级分值不存在统计学差异(P>0.05)。结论腭裂修补术后经过言语康复治疗,嗓音声学参数的听主观评估结果提示患儿嗓音音质有明显改善,临床上应重视腭裂术后言语康复治疗训练内容。Objective To investigate the clinical changes of vocal acoustical parameters GRBAS and speech intelligibility(%)before and after speech rehabilitation in children post operation for cleft palate.Methods Forty cases with speech rehabilitation after cleft palate operation were reviewed and rating score of the vocal acoustical parameters GRBAS(grade,roughness,breathiness,asthenia,strain)with the natural prolonged sound/a/and speech intelligibility(%)before and after speech rehabilitation were analyzed and compared.Results After speech rehabilitation treatment,G,R rating score and speech intelligibility(%)were significantly improved compared with those before treatment.There were significant difference in G,R rating score and speech intelligibility(%)(P<0.05),but there were no significant difference in B,A and S score before and after speech rehabilitation treatment(P>0.05).Conclusion After speech rehabilitation treatment,the results of subjective assessment of vocal acoustical parameters suggest that the voice quality of these children have been significantly improved.More clinical attention should be paid to speech rehabilitation training in children post operation for cleft palate.
