不同根管冲洗剂对根管玷污层清洗效果的体外研究

摘　　要：目的比较不同根管冲洗剂对根管内壁玷污层的清洗效果。方法将32颗离体牙随机分成4组,每组8颗,A组0.9%生理盐水冲洗,B组3%双氧水冲洗,C组3%次氯酸钠冲洗,D组2%氯亚明冲洗。根管预备完成后,扫描电镜观察根管中1/3的玷污层去除情况,参考玷污层分级标准,对数据进行统计学分析,比较各组根管玷污层清洗效果。结果A、B组与C组之间差异有统计学意义(P<0.05),A、B组与D组之间差异有统计学意义(P<0.05),A、B组间差异无统计学意义(P>0.05);C、D组计分标准均低于A、B组,差异有统计学意义(P<0.05),C、D组间差异无统计学意义(P>0.05)。结论使用ProTaper机用镍钛扩大针结合19%EDTA凝胶扩锉根管时,3%次氯酸钠和2%氯亚明对根管中部1/3玷污层的清洗效果优于0.9%生理盐水和3%双氧水。Objective To compare the ability of different endodontic irrigations to remove the smear layer of root canal.Methods A total of 32 isolated teeth were collected and randomly divided into 4 groups(n=8)for different irrigations:group A(0.9%NaCl),group B(3%hydrogen peroxide),group C(3%sodium hypochlorite),and group D(2%chloramine).Whilst the irrigation,root canals preparation was carried out by the ProTaper Nickel-titanium instrument and 19%EDTA.Then after preparation and irrigation,teeth were prepared for SEM(scanning electron microscope)analysis to evaluate the cleaning of the middle third of radicular dentin by determining the presence or absence of smear layer.Results There were no significant differences between group A and B(P>0.05),and between group C and D(P>0.05),while the cleaning effect of group C/D was statistically better than group A/B(P<0.05).Conclusion During root canals preparation with ProTaper Nickel-titanium rotary instrument and 19%EDTA,3%sodium hypochlorite and 2%chloramin are more effective than0.9%NaCl and 3%hydrogen peroxide in the removal of smear layer from the root canal system.
