摘 要:目的探讨颈椎功能不良与颞下颌关节紊乱病(TMD)之间的相关性。方法选取2019年3月~2020年10月之间,在我院确诊的TMD患者186例。采用Pearson线性相关分析和多元线性回归分析颈椎功能障碍指数(NDL)与年龄、颞下颌功能限制量表(JFLS)、数字疼痛评定量表(NPRS)、最大开口度(MMO)、身体质量指数(BMI)之间的相关性。结果所有186例患者中,NDI评分在13~43之间,平均20.4±9.5;JFLS评分在17~91之间,平均47.3±20.2;NPRS评分在1.0~9.0之间,平均5.7±1.8;MMO在21.3~29.8mm,平均24.7±1.0mm;BMI指数在21~32之间,平均25.6±1.8。Pearson相关性分析显示,NDL评分与JFLS、NPRS及BMI评分呈正相关(r=0.905,P<0.05,r=0.726,P<0.05,r=0.539,P<0.05),与MMO评分呈负相关(r=-0.852,P<0.05)。与年龄无明显相关性(r=0.539,P>0.05)。多元线性回归分析发现NDL水平与JFLS评分及MMO相关性最强,NDL与JFLS评分呈正相关,与MMO呈负相关(标准化偏回归系数=0.591、-1.720,P<0.05)。结论NDL水平与TMD患者的JFLS、NPRS、MMO及BMI均相关,并且与JFLS及MMO密切相关。因此,颈椎功能不良可能是TMD发病及病程进展的重要影响因素,改善颈椎功能可以做为防治TMD的新方法。Objective To investigate the correlation between Cervical Vertebra Function and Temporomandibular Disorder.Methods We analyzed clinical data of 186 patients who were diagnosed as TMD in the Second Hospital of Hebei Medical University from March 2019 to October 2020.Pearson linear correlation analysis and multiple linear regression were used to analyze the correlation between cervical spine dysfunction index(NDL)and age,Jaw Functional Limitation Scale(JFLS),Numeric Pain Rating Scale(NPRS),Maximal Mouth Opening(MMO)and Body Mass Index(BMI).Results The NDI score ranged from 13 to 43,with an average of 20.4±9.5;JFLS score was ranged from 17 to 91,with an average of 47.3±20.2;NPRS ranged from 1.0 to 9.0,with an average of 5.7±1.8;MMO was ranged from 21.3 mm to 29.8 mm,with an average of 24.7±1.0 mm;BMI was ranged from 21 to 32,with an average of 25.6±1.8.Pearson correlation analysis showed that NDL score was positively correlated with JFLS,NPRS and BMI(r=0.905,P<0.05,r=0.726,P<0.05,r=0.539,P<0.05),and negatively correlated with MMO score(r=-0.852,P<0.05).There was no significant correlation with age(r=0.539,P>0.05).Multiple linear regression analysis indicated that NDL level had the strongest correlation with JFLS score and MMO score.NDL was positively correlated with JFLS score and negatively correlated with MMO(standardized partial regression coefficient=0.591,-1.720,P<0.05).Conclusion In TMD patients,the level of NDL was correlated with JFLS,NPRS,MMO and BMI.Especially,the NDL was closely related to JFLS and MMO.Therefore,cervical vertebra function may be an important factor in the pathogenesis and progression of TMD.Improving cervical function can be used as a new method for prevention and treatment of TMD.
