摘 要:目的建立一种双侧完全性唇腭裂患儿唇裂术后个性化鼻模的计算机辅助设计及制作方法。方法选取3例无法佩戴常规鼻模的双侧完全性唇腭裂患儿,通过三维摄影测量系统扫描唇裂术后的患儿面部以获取三维原始数据,并重建数字化鼻-唇部模型,三维设计及打印个性化硅胶鼻模以精确匹配鼻唇部结构,并在临床戴用检验其矫治效果。结果三维摄影测量系统20 s内成功获取并重建患儿鼻唇部精准外形,以此为基准设计的个性化鼻模贴合度佳,固位良好,患儿佩戴后无不适感,可以较好地撑起鼻翼,维持鼻部形态。制作鼻模所选取的硅胶材料,生物安全性好,无黏膜刺激等现象。结论通过三维摄影测量系统与三维设计、增材制造技术,设计制造唇腭裂患儿个性化鼻模,是一种简便且安全有效的方式。Objective To establish a new CAD-CAM method to design and manufacture personalized nasal stent after cheiloplasty for infants with complete bilateral cleft lip and palate.Methods Three children of bilateral cleft lip and palate who could not wear standardized nasal stent were enrolled in this study.The patients were scanned by 3dMD facial photogrammetry system to obtain 3D original data,and the digital nasal-labial model was reconstructed.Personalized silicone nasal stent was designed and printed in 3D to accurately match the nasal-labial structure,and its correction effect was tested in clinical practice.Results The original data of nasal-labial area was obtained and 3D model was reconstructed within 20 seconds.The personalized nasal stent designed based on the system had a good fitting degree and retention,the children had no discomfort,and it could hold up the nasal alar and maintain the nasal morphology.The silica gel material has good biological safety without mucosal irritation.Conclusion It is a simple,safe and effective way to manufacture personalized stent for infants with complete bilateral cleft lip and palate by combining 3dMD photography with CAD-CAM technology.
