摘 要:目的探讨结缔组织生长因子(CTGF)在多发性骨髓瘤(MM)组织中的表达变化及其与患者临床病理特征和预后的关系。方法在Oncomine数据库中筛选CTGF在MM组织中的表达情况,分析MM组织与正常浆细胞CTGF的表达差异。从GEO数据库中下载GSE24080数据集,提取初诊MM患者临床病理资料,包括性别、年龄、种族、免疫球蛋白(Ig)亚型、β2微球蛋白(β2M)、C反应蛋白(CRP)、肌酐(Scr)、乳酸脱氢酶(LDH)、白蛋白(Alb)、血红蛋白(Hb)、骨髓抽吸浆细胞比例(ASPC)、骨髓活检浆细胞比例(BMPC)、MRI局灶病变、分子遗传学异常,以及总体生存率(OS)和无事件生存率(EFS)情况。以CTGF相对表达量的中位数为临界值,将患者分为CTGF低表达者与CTGF高表达者,比较二者临床病理特征和预后情况。结果Oncomine数据库中关于MM组织与正常浆细胞CTGF表达差异的研究共3项,符合本研究筛选条件的有2项。在这2项研究中,MM组织CTGF表达均高于正常浆细胞(P均<0.05)。在GSE24080数据集中,CTGF低表达者与CTGF高表达者Hb、BMPC比较差异均有统计学意义(P均<0.05),而性别、年龄、种族、Ig亚型、β2M、CRP、Scr、LDH、Alb、ASPC、MRI局灶病变、分子遗传学异常比较差异均无统计学意义(P均>0.05)。CTGF高表达者5年OS、EFS均高于CTGF低表达者(HR分别为0.71、0.73,95%CI分别为0.53~0.96、0.57~0.93,P均<0.05);Cox回归分析显示,CTGF表达是影响MM患者5年OS、EFS的独立危险因素(P均<0.05)。结论MM组织CTGF高表达,其表达变化与患者临床病理特征无明显相关性,但CTGF高表达者预后较好。CTGF可作为MM患者预后判断的生物学指标。Objective To investigate the expression of connective tissue growth factor(CTGF)in the multiple myeloma(MM)tissues and its association with patients'clinicopathological characteristics and prognosis.Methods We screened the expression of CTGF in MM tissues in the Oncomine database.The expression difference of CTGF between MM and normal plasma cells was searched by using Oncomine database.We downloaded GSE24080 dataset from GEO database and extracted the clinicopathological data of newly diagnosed MM patients,including gender,age,race,immu⁃noglobulin(Ig)subtype,β2 microglobulin(β2M),C-reactive protein(CRP),creatinine(Scr),lactate dehydrogenase(LDH),albumin(Alb),hemoglobin(Hb),bone marrow aspirated plasma cell ratio(ASPC),bone marrow biopsy plas⁃ma(BMPC),MRI focal lesions,and molecular genetic abnormalities,and meanwhile,the overall survival(OS)and event-free survival(EFS)were analyzed.The median relative expression of CTGF was taken as the critical value,and the patients were divided into two groups:the low CTGF expression group and the high CTGF group.The clinicopathological features and prognosis of the two groups were compared.Results There were three studies on the difference of CTGF expression between MM and normal plasma cells in Oncomine database,two of which conformed to the screening condi⁃tions of this study.In these two studies,the expression of CTGF in the MM tissues was higher than that of the normal plas⁃ma cells(all P<0.05).There were significant differences in Hb and BMPC between the low CTGF group and high CTGF group in GSE24080 dataset(all P<0.05),but there were no significant differences in gender,age,race,Ig subtype,β2M,CRP,Scr,LDH,Alb,ASPC,MRI focal lesions,or molecular genetic abnormalities(all P>0.05).The 5-year OS and EFS were higher in patients with high CTGF expression than in patients with low CTGF expression(HR=0.71,0.73;95%CI:0.53-0.96,0.57-0.93;all P<0.05).Cox regression analysis showed that CTGF expression was an independent risk factor for 5-year OS and EFS in patients wit
