摘  要:目的探讨瞬时感受器电位离子通道蛋白4(transient receptor potential vanilloid 4,TRPV4)和P38α在大鼠背根神经节持续受压(chronic compression of dorsal root ganglion,CCD)致神经病理性疼痛中的作用。方法采用健康成年雄性Wistar大鼠,随机分为空白组、CCD组及CCD+钌红组。空白组及CCD手术组于术前1天、术后1天、7天和21天测量机械刺激痛阈(paw withdrawal mechaical threshold,PWMT),CCD+钌红组于术后第7天鞘注钌红前后测量PWMT;采用RT-PCR技术检测各组大鼠术侧背根神经节(dorsal root ganglion,DRG)内TRPV4和P38α基因表达变化。结果与空白组相比,CCD组大鼠术侧PWMT明显下降(P<0.001),CCD7 d组下降最明显(P<0.001);同侧DRG内TRPV4和P38α基因表达均明显升高(P<0.001),且CCD7 d组升高最明显(P<0.001);鞘注钌红2 h后,CCD组大鼠术侧PWMT明显升高(P<0.001),同侧DRG内P38α基因表达明显降低(P<0.001)。结论大鼠CCD术后术侧DRG内TRPV4和P38α基因表达变化与PWMT变化相一致,TRPV4-P38α信号通路可能参与CCD所致神经病理性疼痛的病理过程。Objective:To investigate the role of transient receptor potential vanilloid 4(TRPV4)and P38αin neuropathic pain induced by chronic compression of dorsal root ganglion(CCD)in rats.Methods:Healthy adult male Wistar rats were randomly divided into blank group,CCD group and CCD+ruthenium red group.Paw withdrawal mechaical threshold(PWMT)was measured 1 day before surgery,1 day,7 days,and 21 days after surgery in the blank group and CCD surgery group.The CCD+ruthenium red group were injected with ruthenium red on the 7th day after surgery,and the PWMT was measured before and after that;RT-PCR was used to detect the expressions of TRPV4 and P38αgenes in the dorsal root ganglion(DRG)in each group of the rats.Results:Compared with the blank group,the PWMT of the CCD group decreased significantly on the operation side(P<0.001),and the CCD 7-day group had the most significant decrease(P<0.001).The expressions of TRPV4 and P38αgenes in the ipsilateral DRG were significantly increased(P<0.001),and the CCD 7-day group had the most significant increase(P<0.001).After 2 hours of ruthenium red injection,the PWMT of the CCD group significantly increased(P<0.001),while the P38αgene expression in the ipsilateral DRG was significantly reduced(P<0.001).Conclusion:The expressions of TRPV4 and P38αgenes in the DRG of the operative rats after CCD are consistent with the changes of PWMT.The TRPV4-P38αsignal pathway may be involved in the pathological process of neuropathic pain caused by CCD.
