不同溶液在放射性皮炎换药中的效果研究

摘　　要：目的观察灭菌注射用水与0.9%氯化钠两种不同溶液在鼻咽癌急性Ⅲ度及以上放射性皮炎换药中的作用。方法回顾性分析我科2018年6月至2019年9月首诊鼻咽癌行放射治疗后发生急性Ⅲ度及以上放射性皮炎患者64例,按治疗方法不同分为观察组和对照组,每组各32例。观察组患者换药时采用的是灭菌注射用水;对照组换药时采用的是0.9%氯化钠溶液。对两组患者换药过程及换药后2 h疼痛程度及创面开始愈合时间进行分析。结果观察组换药过程中,轻度疼痛例数高于对照组,中度和重度疼痛例数低于对照组,差异有统计学意义(Z=10.604,P<0.05);换药2 h后,观察组无痛及轻度疼痛患者例数高于对照组,中度及重度疼痛例数低于对照组,差异有统计学意义(Z=13.041,P<0.05);观察组患者创面开始愈合时间明显早于对照组,差异有统计学意义(P<0.05)。结论放射性皮炎患者换药时采用灭菌注射用水溶液,创面疼痛轻,愈合快,值得推广。Objective To observe the effect of sterilized water for injection and 0.9%sodium chloride in dressing change of acute radiodermatitis gradeⅢor above of nasopharyngeal carcinoma.Methods A total of 64 patients with acute radiodermatitis gradeⅢor above after radiotherapy for nasopharyngeal carcinoma first diagnosed in our department from June 2018 to September 2019 were analyzed retrospectively.According to the treatment methods,they were divided into the observation group(n=32)and the control group(n=32).Patients of the observation group were treated with sterilized water for injection,while the control group was treated with 0.9%sodium chloride solution.The data of both groups of patients during and 2 hours after the dressing change in the pain degree and the time of wound healing were analyzed.Results In the process of dressing change,the number of mild pain cases in the observation group was higher than that in the control group,while the number of moderate and severe pain cases was lower than that in the control group,with statistically significant differences between the two groups(Z=10.604,P<0.05).Two hours after dressing change,the number of painless and mild pain cases in the observation group was higher than that in the control group,while the number of moderate and severe pain cases was lower than that in the control group,with statistically significant differences(Z=13.041,P<0.05).The time of wound healing in the observation group was significantly earlier than that in the control group,with statistically significant difference(P<0.05).Conclusion Sterilized water for injection is used in patients with radiodermatitis in dressing change,which has mild wound pain and quick healing,and is worthy of promotion.
