艾司奥美拉唑联合替普瑞酮治疗老年巨大胃溃疡的疗效及对TNF-α、IL-6和IL-8的影响探讨

摘　　要：目的探讨艾司奥美拉唑联合替普瑞酮治疗老年巨大胃溃疡的效果及对肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-8(IL-8)的影响。方法98例老年巨大胃溃疡患者,随机分为研究组和对照组,各49例。对照组采用艾司奥美拉唑治疗,研究组采用艾司奥美拉唑联合替普瑞酮治疗。比较两组患者的治疗效果、不良反应发生情况、视觉模拟评分法(VAS)评分及TNF-α、IL-6、IL-8水平。结果研究组总有效率91.84%明显高于对照组的75.51%,差异具有统计学意义(P<0.05)。治疗前,两组VAS评分比较差异无统计学意义(P>0.05);治疗后,两组VAS评分均低于治疗前,且研究组VAS评分(1.83±0.27)分低于对照组的(3.01±0.44)分,差异具有统计学意义(P<0.05)。治疗前,两组IL-6、IL-8和TNF-α水平比较差异无统计学意义(P>0.05);治疗后,两组IL-6、IL-8、TNF-α水平均低于治疗前,且研究组均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论采用艾司奥美拉唑联合替普瑞酮治疗老年巨大胃溃疡疗效显著且安全,能有效降低炎症因子水平,缓解腹痛情况,值得临床推广应用。Objective To discuss the effect of esomeprazole combined with teprenone in the treatment of elderly patients with giant gastric ulcer and its influence on tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-8(IL-8).Methods A total of 98 elderly patients with giant gastric ulcer were randomly divided into research group and control group,with 49 cases in each group.The control group was treated with esomeprazole,and the research group was treated with esomeprazole combined with teprenone.The therapeutic effect,occurrence of adverse reactions,visual analogue scale(VAS)score,and levels of TNF-α,IL-6 and IL-8 were compared between the two groups.Results The total effective rate 91.84%of the research group was obviously higher than 75.51%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in VAS score between the two groups(P>0.05).After treatment,the VAS score of the two groups was lower than that before treatment,and the VAS score(1.83±0.27)points of the research group was lower than(3.01±0.44)points of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in levels of IL-6,IL-8 and TNF-αbetween the two groups(P>0.05).After treatment,the levels of IL-6,IL-8 and TNF-αof the two groups were all lower than those before treatment,and the research group was lower than the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion Application of esomeprazole and teprenone is effective and safe in the treatment of elderly patients with giant gastric ulcer,and it can effectively reduce the level of inflammatory factors and relieve abdominal pain.It is worthy of clinical promotion and application.
