摘 要:目的探讨低剂量螺旋CT在原发性肺癌患者诊断时影像学特征及手术治疗指导价值研究。方法选取2018年8月至2020年3月潜江市中心医院收治的原发性肺癌患者96例,所有患者均应用常规剂量螺旋CT和低剂量螺旋CT扫描。比较常规剂量螺旋CT和低剂量螺旋CT扫描钙化、空洞、毛刺、分叶情况、原发性肺癌的有效放射剂量以及检出率、诊断符合率、图像质量。结果常规剂量螺旋CT和低剂量螺旋CT扫描钙化、空洞、毛刺、分叶情况比较,差异无统计学意义(P=0.058、0.061、0.063、0.068);低剂量螺旋CT扫描对原发性肺癌检出率高于常规剂量螺旋CT,差异有统计学意义(P=0.042),低剂量螺旋CT有效放射剂量低于常规剂量螺旋CT,差异有统计学意义(P=0.033);低剂量螺旋CT误诊率低于常规剂量螺旋CT,差异有统计学意义(P=0.031),诊断符合率高于常规剂量螺旋CT,差异有统计学意义(P=0.036);常规剂量螺旋CT和低剂量螺旋CT图像质量,差异无统计学意义(P=0.058)。结论原发性肺癌患者诊断时,低剂量螺旋CT可以给予更多的手术治疗指导价值,同时提高影像学特征,临床上应当进一步推广应用。Objective To investigate the imaging features and guiding value of surgical treatment of low dose spiral CT in diagnosis of patients with primary lung cancer.Methods A total of 96 patients with primary lung cancer admitted to Qianjiang Central Hospital from August 2018 to March 2020 were selected,and all patients were scanned by conventional dose or low dose spiral CT scanning.The calcification,cavity,burr,lobulation conditions,effective radiation dose of primary lung cancer,as well as detection rate,diagnostic coincidence rate and image quality between conventional dose spiral CT and low dose spiral CT scanning were compared.Results There was no statistically significant difference in calcification,cavity,burr and lobulation conditions between conventional dose spiral CT and low dose spiral CT scanning(P=0.058,0.061,0.063 and 0.068).The detection rate of primary lung cancer by low dose spiral CT was higher than that by conventional dose spiral CT,with statistically significant difference(P=0.042).The effective radiation dose of low dose spiral CT was lower than that by conventional dose spiral CT,with statistically significant difference(P=0.033).The misdiagnosis rate of low dose spiral CT was significantly lower than that of conventional dose spiral CT,with statistically significant difference(P=0.031).And the diagnostic coincidence rate was significantly higher than that of conventional dose spiral CT,with statistically significant difference(P=0.036).There was no statistically significant difference in image quality between conventional dose spiral CT and low dose spiral CT(P=0.058).Conclusion In the diagnosis of primary lung cancer,low dose spiral CT can provide more guiding value for surgical treatment and improve imaging features,which should be further promoted and applied in clinical practice.
