摘 要:目的探究显微外科技术切除听神经瘤面听神经解剖和功能保留的影响。方法对江门市中心医院神经外科2017年3月至2019年8月收治的66例术后病理确诊为听神经瘤患者临床资料进行回顾性分析。结果肿瘤全切除62例(93.94%),次全切除4例(6.06%);面神经解剖保留63例(95.45%),听神经解剖保留17例(25.76%)。术后1年面神经功能与术后1周比较,差异无统计学意义(P>0.05);术后1年听力情况与术后1周比较,差异无统计学意义(P>0.05)。66例患者术后发生颅内感染4例(6.06%)、肺部感染2例(3.03%)、术区出血1例(1.52%)、脑积水1例(1.52%)、暴露性角膜炎1例(1.52%),合计发生9例(13.64%)。结论对听神经瘤患者应用显微外科技术切除肿瘤,听神经瘤切除率较高,可获得良好面听神经解剖保护与听功能保留。Objective To investigate the impacts of microsurgical resection of acoustic neuroma and preservation of facial acoustic nerve function.Methods The clinical data of 66 patients with acoustic neuroma diagnosed by pathology after operation admitted to Department of Neurosurgery,Jiangmen Central Hospital from March 2017 to August 2019 were retrospectively analyzed.Results Total tumor resection was performed in 62 cases(93.94%)and subtotal tumor resection in 4 cases(6.06%).Facial nerve was anatomically preserved in 63 cases(95.45%)and auditory nerve in 17 cases(25.76%).There was no statistically significant difference in facial nerve function between 1 year after operation and 1 week after operation(P>0.05).There was no statistically significant difference in hearing between 1 year after operation and 1 week after operation(P>0.05).There were 4 cases(6.06%)of intracranial infection,2 cases(3.03%)of pulmonary infection,1 case(1.52%)of bleeding in operation area,1 case(1.52%)of hydrocephalus and 1 case(1.52%)of exposure keratitis after operation,with a total of 9 cases(13.64%).Conclusion The removal rate of acoustic neuroma is high when microsurgical technique is applied to patients with acoustic neuroma,and good anatomical protection of facial acoustic nerve and preservation of auditory function can be obtained.
