后腹腔镜联合腹部小切口根治性肾输尿管切除治疗上尿路肿瘤的手术技巧与疗效观察

摘　　要：目的:分析后腹腔镜联合腹部小切口根治性肾输尿管切除治疗上尿路肿瘤的效果。方法:选取2016年3月~2018年9月在本院行后腹腔镜联合腹部小切口治疗上尿路肿瘤的30例患者作为研究对象,所有患者接受手术治疗,分析治疗手术技巧及治疗效果。结果:手术全部成功,术中与术后没有发生并发症,手术时长(128.41±16.28)min;术中出血量达到(78.89±18.65)mL,所有患者都没有输血。手术后肠功能恢复时长(28.45±9.23)h;住院时长(7.91±2.24)d。手术后病理结果显示所有患者都是尿路上皮癌,其中肾盂癌21例,输尿管癌9例;随访6~24个月,都没有发生肿瘤复发与转移。结论:后腹腔镜联合腹部小切口根治性肾输尿管切除术,疗效显著,可以有效改善病情,术后不易复发,具有较高的安全可靠性,可以较好改善患者的生活品质。Objective:To analyze the effect of retroperitoneal laparoscopy combined with small abdominal incision radical nephroureterectomy in the treatment of upper urinary tract tumors.Methods:Thirty patients who underwent retroperitoneal laparoscopy combined with small abdominal incision in the treatment of upper urinary tract tumors in our hospital from March 2016 to September 2018 were selected as the research objects.All patients received surgical treatment,and the surgical techniques and treatment effects were analyzed.Results:The operations were all successful.There were no complications during and after the operation.The operation time was(128.41±16.28)min;the intraoperative blood loss reached(78.89±18.65)mL.All patients had no blood transfusion.The length of intestinal function recovery after surgery was(28.45±9.23)h;the length of hospitalization was(7.91±2.24)d.The pathological results after surgery showed that all patients were urothelial cancers,including 21 cases of renal pelvic cancer and 9 cases of ureteral cancer.There was no tumor recurrence or metastasis after a follow-up of 6 to 24 months.Conclusion:Retroperitoneal laparoscopy combined with small abdominal incision radical nephroureterectomy has a significant effect,can effectively improve the condition,is not easy to relapse after surgery,has high safety and reliability,can better improve the quality of life of patients.
