摘  要:目的:探讨CT引导下改良双通道微创钻孔引流术治疗大面积脑出血临床效果。方法:选择2017年2月~2019年6月本院收治的74例大面积脑出血患者分为观察组和对照组各37例。对照组给予传统钻孔引流,而观察组给予CT引导改良双通道微创状孔引流术。统计两组患者术后的再出血、肺部感染、脑积水以及上消化道出血等并发症情况及术后的生活质量。结果:观察组的术后并发症发生率5.41%,对照组21.62%,差异有统计学意义(P<0.05);两组术后的5级生活质量比较,P>0.05;观察组术后1~2级生活质量占比高于对照组,3~4级低于对照组,有统计学差异(P<0.05)。结论:针对大面积脑出血患者采用CT引导下改良双通道微创钻孔引流术方案治疗可显著提升疗效,有利于降低患者的术后并发症发生率并改善预后。Objective:To explore the clinical effect of CT guided modified double channel minimally invasive drilling and drainage in the treatment of massive cerebral hemorrhage.Methods:The 74 patients with extensive cerebral hemorrhage admitted to our hospital from February 2017 to June 2019 were divided into observation group and control group with 37 cases each.The control group was given traditional drilling and drainage,while the observation group was given CT-guided modified dual-channel minimally invasive drainage.The postoperative complications such as rebleeding,lung infection,hydrocephalus,upper gastrointestinal hemorrhage and postoperative quality of life of the two groups of patients were counted.Results:The postoperative complication rate of the observation group was 5.41%and that of the control group was 21.62%,the difference was statistically significant(P<0.05);the comparison of the five-level quality of life between the two groups after surgery,P>0.05;the proportion of quality of life of the observation group was higher than that of the control group at grades 1 to 2 after operation,and the quality of life at grades 3 to 4 was lower than that of the control group.There was a statistical difference(P<0.05).Conclusion:CT guided modified double channel minimally invasive drilling and drainage can significantly improve the curative effect,which is beneficial to reduce the incidence of postoperative complications and improve the prognosis of patients.
