阴道镜联合LEEP在宫颈癌前病变患者中的疗效观察及对HPV清除率的影响研究

摘　　要：目的:探究阴道镜联合宫颈环形电切术(LEEP)在宫颈癌前病变患者中的效果及对人乳头瘤病毒(HPV)清除率的影响。方法:回顾性分析2018年1月~2020年1月本院术后病理检查确诊为宫颈上皮内瘤变(CIN)的72例患者的临床资料,均行阴道镜下病理活检及阴道镜下LEEP治疗,观察治疗效果。结果:治愈70例(97.22%),残留1例(1.39%),术后12个月复发1例(1.39%);术后12个月,患者人乳头瘤病毒(HPV)阳性率为1.39%,低于术前(100.00%)、术后6个月(11.11%),差异显著(P<0.05),术后6个月患者HPV阳性率低于术前,差异显著(P<0.05)。结论:阴道镜联合LEEP治疗能够取得较高的HPV清除率,应用于宫颈癌前病变患者中疗效确切。Objective:To study the impact of colposcopy plus LEEP(loop electro-surgical excision procedure)on the HPV(human papillomavirus)resection rates of cervical intraepithelial neoplasia(CIN)patients.Methods:Clinical data about 72 CIN patients of receiving the pathological biopsy from January 2018 to January 2020 in our hospital was retrospectively analyzed.According to operation indications,patients took the colposcopy and LEEP.Operation conditions and postoperative recovery conditions(operation time,intraoperative bleeding volume and postoperative wound healing time),operation effects(cured,residual,recurrent)and HPV positive rates before surgery,after 6 months and 12 months of surgery were observed.Results:The operation time,intraoperative bleeding volume and postoperative wound healing time was(8.26±1.13)min,(11.22±3.42)mL and(50.26±5.86)d.There were 70 cured cases(97.22%),one residual case(1.39)and one recurrent case(1.39%)after 12 months of surgery;after 12 months of surgery,HPV positive rate(1.39%)was significantly lower than before(100.00%)and those after 6 months of surgery(11.11%)(P<0.05);HPV positive rate after 6 months of surgery was significantly lower than before(P<0.05).Conclusion:Colposcopy combined with LEEP treatment can achieve a higher HPV clearance rate,and it has a definite effect when applied to patients with cervical precancerous lesions.
