摘 要:目的研究不同临床分期的甲状腺癌中TP53和Wnt-5a的表达特点,探讨其在甲状腺癌中表达的临床意义。方法通过免疫组织化学技术对50例甲状腺癌组织石蜡标本和10例癌旁组织石蜡标本中TP53和Wnt-5a的表达进行检测,采用RT-PCR技术对23例手术切除的新鲜甲状腺癌组织和10例癌旁组织中TP53mRNA和Wnt-5a mRNA表达进行检测,分析其差异性。结果TP53蛋白、mRNA在甲状腺癌组织中表达高于癌旁组织,在甲状腺癌Ⅲ~Ⅳ期的表达高于Ⅰ~Ⅱ期(P<0.05);Wnt-5a蛋白、mRNA在甲状腺癌表达低于癌旁组织,在甲状腺癌Ⅰ~Ⅱ期的表达高于Ⅲ~Ⅳ期(P<0.05);TP53和Wnt-5a基因的表达呈负相关(r=-0.720,P<0.05)。结论TP53和Wnt-5a基因的表达参与了甲状腺癌的发生发展,同时Wnt-5a激活的信号转导途径可能在临床甲状腺癌的治疗中具有辅助作用,二者联合检测对临床手术方式、术后治疗策略选择、预后评估具有重要的价值。Objective To study the expression characteristics of TP53 and Wnt-5a in thyroid carcinoma of different clinical stages,and to explore the clinical significance of their expression in thyroid carcinoma.Methods The expression of TP53 and Wnt-5a in paraffin specimens from 50 thyroid carcinoma tissues and 10 paracarcinoma tissues were detected by immunohistochemistry.RT-PCR technology was used to detect the expression of TP53 mRNA and Wnt-5a mRNA in 23 cases of fresh thyroid carcinoma tissues and 10 cases of adjacent tissues to analyze their differences.Results The expression of TP53 protein and mRNA in thyroid carcinoma tissue was higher than that in adjacent tissues,and the expression in thyroid carcinoma stage III-IV was higher than that in stage I-II(P<0.05);The expression of Wnt-5a protein and mRNA in thyroid carcinoma was lower than that in adjacent tissues,and the expression in thyroid carcinoma stage I-II was higher than that in stage III-IV(P<0.05);The expression of TP53 and Wnt-5a gene was negatively correlated(r=-0.720,P<0.05).Conclusion The expression of TP53 and Wnt-5a genes are involved in the occurrence and development of thyroid carcinoma.At the same time,the signal transduction pathway activated by Wnt-5a may play an auxiliary role in the treatment of clinical thyroid carcinoma.The combined detection of the two has important value for clinical operation methods,postoperative treatment strategy selection,and prognosis evaluation.
