不同频率电针对老年结直肠癌根治术患者术中应激反应及术后免疫功能、炎性反应的影响

摘　　要：目的观察不同频率电针对老年结直肠癌根治术患者术中应激反应及术后免疫功能、炎性反应的影响。方法将80例老年结直肠癌根治术患者随机分为2组,均于术前1 d、术前30 min行电针治疗。治疗组40例采用疏密波,频率2 Hz/100 Hz;对照组40例采用连续波,频率2 Hz。此后在常规全身麻醉下行结直肠癌根治术。比较2组主要临床评价指标;比较2组术前、术中血清皮质醇(COR)、β-内啡肽(β-EP)含量变化;比较2组术前1 d、术中、术后1 d CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)变化;比较2组术前1 d、术中、术后1 d血清肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、IL-10含量变化。结果治疗组术后肠鸣音恢复时间、首次排气时间、首次排便时间、首次进食时间及住院时间均短于对照组(P<0.05)。2组术中COR、β-EP含量均较本组术前升高(P<0.05),术中治疗组COR、β-EP含量均低于对照组(P<0.05)。2组术后1 d CD4^(+)/CD8^(+)均较本组术中升高(P<0.05),且治疗组高于对照组(P<0.05)。2组术中、术后1 d CD4^(^(+))/CD8^(^(+))均较本组术前降低(P<0.05);治疗组术中CD4^(+)/CD8^(+)高于对照组(P<0.05)。治疗组术中CD3^(+)较本组术前1 d降低(P<0.05);对照组术中、术后1 d CD3^(+)较本组术前1 d降低(P<0.05)。2组术后1 d TNF-α、IL-6、IL-10含量均较本组术中升高(P<0.05),术后1 d治疗组TNF-α、IL-6、IL-10含量均低于对照组(P<0.05)。2组术中、术后1 d TNF-α、IL-6、IL-10含量均较本组术前升高(P<0.05);治疗组术中TNF-α、IL-6、IL-10含量均低于对照组(P<0.05)。结论对老年结直肠癌根治术患者进行电针治疗,2 Hz/100 Hz电针频率可更好地降低术中应激反应,调节术后免疫功能,减轻炎性反应。Objective To observe the effects of different frequency electroacupuncture on intraoperative stress response,postoperative immune and inflammatory response in elderly patients with colorectal cancer undergoing radical resection.Methods Eighty cases of elderly patients with colorectal cancer undergoing radical resection were randomly divided into two groups.All patients were treated with electroacupuncture in 1 day before the operation and 30 minutes before the operation,respectively.40 cases in the treatment group were treated with dense wave,with frequency 2 Hz/100 Hz,and 40 cases in the control group of with continuous wave,frequency of 2 Hz.After this,radical resection of colorectal cancer was performed under general anesthesia.The main clinical evaluation indexes in the two groups were observed,the changes of plasma cortisol(COR)beta-endorphin(β-EP)levels before and during operation were compared between the two groups.The changes of CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+),serum tumor necrosis factorα(TNF-α),interleukin-6(IL-6)and IL-10 levels were compared on the fist day before operation,during operation,and one day after operation.Results After operation,the recovery time of bowel sounds,the first exhaust time,the first defecation time,the first eating time and hospitalization time in the treatment group were shorter than those in the control group(P<0.05).During the operation,the contents of COR andβ-EP in the two groups were higher than those before operation(P<0.05),and those in the treatment group were lower than in the control group(P<0.05);on the first day after operation,the level of CD4^(+)/CD8^(+)was higher than that during operation in the two groups(P<0.05),with the increase more significant in the treatment group(P<0.05);that in both groups during operation and on the first day after operation was lower than before operation(P<0.05),CD4^(+)/CD8^(+)in the treatment group during operation was higher than that in the control group(P<0.05).The level of CD3^(+)in the treatment group during op
