摘　　要：随着医学时代的发展,人类以生存为核心的模式已经逐渐转变为以提高生活质量为核心的模式,功能性疾病日趋得到人们的关注;医学模式也从“以疾病为中心”转变为“以人为中心”,整合医学(HIM)理论为医学界所认同和推广。盆底功能障碍性疾病患者是一个不可忽视的群体,盆底功能障碍是由于盆底支持结构缺陷、损伤、功能退化或障碍所致,包括尿失禁、排便障碍、盆腔脏器脱垂、肛门直肠黏膜脱垂、性功能障碍和慢性盆腔痛等为常见表现的疾病证候群。受传统观念和对疾病认识不足等因素的影响,许多患者存在“讳疾忌医”、“延迟诊治”和“羞于表达”的情况。这类疾病,病因复杂、病程漫长,症状上存在学科交叉重叠,涉及前、中、后盆及心理、康复、营养、中医、健康管理等多学科问题,盆底疾病多学科诊疗已成为发展趋势和医学共识。我院基于HIM理论探索和实践整合盆底医学模式,取得一定的成绩和突破。With the development of the medical era,the focus of healthcare has gradually shifted from human survival to improving the quality of life,and functional diseases are getting more and more attention.The medical model also has been changed from“disease centered”to“human centered”.Patients with pelvic floor dysfunction(PFD)are a group that cannot be ignored.PFD is caused by structural defects,injuries,functional degradation,or disorders of pelvic floor support structures.Its major symptoms include urinary incontinence,defecation disorders,pelvic organ prolapse,anorectal mucosal prolapse,sexual dysfunction,and chronic pelvic pain.Influenced by traditional concepts and insufficient knowledge of the disease,many patients suffer from“taboo against medical treatment”“delayed diagnosis and treatment”and“shy of expression”.PFD has complex etiology and a long course,and its symptoms often overlap with other diseases.It involves anterior,middle,and posterior pelvic problems and multi-disciplinary issues such as psychology,rehabilitation,nutrition,Chinese medicine,and health management.Multi-disciplinary diagnosis and treatment of PFD have become a development trend and medical consensus.Based on the theory of HIM,our hospital explored and practiced the integrated pelvic floor medical model,and made some achievements and breakthroughs.
